How to refer a patient to an anticoagulant clinic
If you are discharging a patient who is on Oral Anticoagulation Therapy
(whether the therapy was initiated on the ward or otherwise) they need to be referred to an anticoagulation clinic.
If the patient’s GP is based in Alderley Edge, Bollington, Chelford, Congleton,
Disley, Handforth, Holmes Chapel, Knutsford, Macclesfield, Poynton, or
Wilmslow then the patient should be referred to the Macclesfield anticoagulant clinic following the procedure for local patients below (steps 1-3). If the patient’s GP isn’t based in one of the areas above then follow the procedure for out-of-area patients (steps a-d).
Procedure for local patients
1. Fax fully completed anticoagulant referral form through to x.1198
2. Telephone x.1836 to make an appointment for the patient
3. Inform patient of clinic appointment
If there is nobody available in the office please leave a message on our answerphone and we will contact the patient with details of their clinic appointment.
Procedure for out-of-area patients a.)
Telephone patient’s GP surgery to find out which Trust they have their anticoagulation contract with. b.) Telephone relevant anticoagulant service to arrange an appointment and check that one of our referral forms is acceptable to fax through. c.) Fax referral through to relevant anticoagulant service. d.) Inform patient of clinic appointment.
