Cleveland Clinic Temporary ID/Badge Process TERMS USED IN PROCESS Temporary = Non-Cleveland Clinic employee requesting temporary badge Supervisor/Administrator = Cleveland Clinic employee administering on-boarding of temporary Temporary Number = Numbers (begin with C or T) that are assigned for one year to individuals classified as non-Cleveland Clinic Employees (ex. Contractors, Students, Vendors). Badges have blue background. PAPERWORK Temporary: Supervisor/Admin: DETAILS: TB Results (from within the last six months) are needed if the individual will be in a Patient Care Area. - Patient Care Area is defined as being “an area that an individual will or may come into contact and/or in the same vicinity with patients of the Cleveland Clinic.” To schedule a TB test with Cleveland Clinic see attachment “Regional Occupational Medicine Services”. NEXT STEP: Complete and return required paperwork provided by Cleveland Clinic Supervisor/Administrator DIRECTIONS: If individual had a temporary badge (blue or green) previously please mark renewal and state the number if known. First and last name given must match identification temporary will use to verify identity when retrieving badge (Driver License/State Issued ID/Social Security Card). Do not use abbreviations for any fields. All badges can have a maximum expiration date of one year. (15 days may be added to the date paperwork is submitted) - Example: Paperwork submitted = 9/5/2010 Latest Expiration Date = 9/20/2011 The CC Supervisor will receive confirmation email unless the Contact fields on the Request form are filled. If the temporary is an independent contractor, state that in the Company/School field on the Request form. Forms must be submitted by the Cleveland Clinic Supervisor or by Cleveland Clinic employee designated by supervisor. Normal processing time is 2-4 business days depending on volume. This is not a guaranteed processing time. - Requests are processed in order as received, we are unable to accommodate rush requests. Revised 2/10/2011 COMET Supervisor/Admin: Temporary: DIRECTIONS: Once processing is complete the Cleveland Clinic Supervisor indicated on the ID Request Form (or the Contact if one is given) will receive a confirmation email from COMET including the temporary’s Temporary ID Number and instructions for COMET. - This indicates that processing for the request is completed. - The confirmation email can only be directed to one individual. COMET modules are determined by patient contact and the profile questionnaire. NEXT STEP: Forward confirmation email to temporary. Make sure they know that the email includes the link for COMET and instructions to log in. DIRECTIONS: After logging into COMET, complete the profile which will determine what modules will need to be completed. Complete all modules under the To Do tab in order to receive ID Badge. If any modules do not fit role re-examine the profile. NEXT STEP: Once COMET has been completed, print the ALL COURSES tab and take with you to the ID Badge Office. RECEIVING ID BADGE Temporary: Supervisor/Admin: Revised 2/10/2011 DIRECTIONS: Bring identification, a print out of the ALL COURSES screen from COMET, and the Background Release Form to the ID Badge Office. ID Badge offices are located: - Main Campus: JJN Building (7:00am-4:15pm) 1956 E. 93rd Street, Cleveland, Ohio 44106 - CCAC: Building 1 (Hours 8:00am-4:00pm) 3050 Science Park Drive, Beachwood, Ohio 44122 Identification can be Driver License/State Issued ID/or Social Security Card. (Must match first and last name submitted to Human Resources) Replacement of a lost badge is a fee of $20 paid to ID Badge when picking up replacement. DIRECTIONS: For additional information including receiving Key/Computer/Epic access and how to proceed for temporary terminations and renewals see sections within this document that pertain to subject matter. Badges not retrieved within two months after paperwork was submitted will need to repeat process from start. KEY/COMPUTER/EPIC ACCESS Supervisor/Admin: DIRECTIONS: To have additional access granted to a badge the Key Control Requisition form needs to be completed and submitted to Key Control. - Key Control Requisition form can be found on the intranet For computer/Epic access the supervisor needs to complete a MyTech request with Information Technology located on the intranet. NAME CHANGE Supervisor/Admin: Temporary: DIRECTIONS: Complete Name Change for Temporary Form and submit to Human Resources. NEXT STEP: Supervisor will be notified once change has been processed, at that time temporary can get new ID Badge. DIRECTIONS: Identification needs to be shown to ID Badge in order to receive new ID Badge. Identification must match first and last name submitted to Sharepoint Administrator. - Identification can be Driver License/State Issued ID/or Social Security Card. BADGE RENEWAL Temporary & Supervisor/Admin: DIRECTIONS: ID Badges will expire annually unless shorter duration is given. ID number issued to temporary will remain the same. Some modules in COMET need to be renewed annually and must be completed prior to receiving new badge. NEXT STEP: Renewal process is the same as a new request. TERMINATION Supervisor/Admin: DIRECTIONS: At the end of a temporary’s time/duration supervisor should collect badge and send to ID Badge Office. Other Cleveland Clinic property should go to respective departments. For individuals ending their time/duration prior to the expiration date on their ID Badge, notify Human Resources.
