Translation Change Request Form
In order for the research supporting the reliability and validity of the measure to apply to its
translation, the translation must be as close as possible to the original English version. Therefore,
translations that change an item to make it less formal or alter the original intent of a question are not
acceptable. However, we do want to correct any inaccuracies in the translations. Therefore, if you believe
a translation has grammatical errors or is incorrect, please complete the following and submit to Molly
Brunk, Ph.D., Director of Quality Assurance, at molly.brunk@mstservices.com.
