The following form is called the FEA which stands for “Field Experience Assessment.” This form was created so you, the Host Mentor/Supervisor, can assess and document the performance level exhibited by the candidate serving in your school. Please fill out the FEA and return it to the candidate so he/she can scan and submit it to the online data entry assessment system called Livetext. The form is divided into three parts: knowledge, skills, and dispositions. The Knowledge section of the form assesses the candidate’s content knowledge and competency in written communication. The Skills section of the form assesses the candidate’s ability to interact with others, plan, and instruct. The Disposition section of the form assesses the candidate’s evidenced behaviors.
