Dear Host Mentor/Supervisor:
The Liberty University School of Education is so grateful that you are participating with us to provide a
field experience that will be rich and meaningful for the LU licensure candidate. We, in turn, hope that
the candidate will also be able to provide you with assistance and experiences that will be beneficial
both to you and your students.
The following form is called the FEA which stands for “Field Experience Assessment.” This form was
created so you, the Host Mentor/Supervisor, can assess and document the performance level exhibited
by the candidate serving in your school. Please fill out the FEA and return it to the candidate so he/she
can scan and submit it to the online data entry assessment system called Livetext.
The form is divided into three parts: knowledge, skills, and dispositions.
The Knowledge section of the form assesses the candidate’s content knowledge and competency in
written communication.
The Skills section of the form assesses the candidate’s ability to interact with others, plan, and instruct.
The Disposition section of the form assesses the candidate’s evidenced behaviors. If you need further
clarification of the “disposition definitions” on the form, please consult the table below:
