Intensive Care Registrar

COLLECTIVE EMPLOYMENT CONTRACT
The following run description is in accordance with the conditions set out in the
current Resident Doctors’ Association & District Health Boards Multi Employer
Collective Agreement.

TRAINING POST
The Registrar position is recognised as a training post by the Joint Faculty of
Intensive Care Medicine for the Intensive care component of ANZCA Training. As
such, all stipulations specified by the Faculty with regard to supervision, assistance,
equipment and other standards are complied with. The run is also accredited for
ACEM training for six months. It has previously been accredited for the C6
component of JFICM training, but needs to be applied for on an individual basis.

CLINICAL RESPONSIBILITIES and WORK SCHEDULES

Other Consultants
Clinical responsibilities to include:
The registrar will work as one of 5 registrars on the team
Care of patients in the Intensive Care Unit and High Dependency Unit under the
supervision of the Consultant of the day.
Registrars are to be available for Air Ambulance activities. This responsibility is
subject to sub clause 6.7 of the current Collective Employment Agreement.
Daytime immediate responsibility is to the consultant of the day in the Intensive Care
Unit. Out of hours to the on call consultant for Intensive Care.

PATIENT CARE and SERVICE DELIVERY RESPONSIBILITIES
As determined by the Directors of Intensive Care, having regard to the service needs
and the requirement of the specialty.

PROVISION FOR TRAINING / EDUCATION
Adequate provision to be made for degree of FANZCA/FJFICM/FACEM (as
appropriate) as well as ongoing education and development of skills. Provisions:

An adequate degree of supervision, for Intensive Care as per document IC- 1

Education in the Intensive Care Unit on the floor as time allows.

Reserved time during normal working week for tutorials and in-service
education - minimum of three hours weekly.

2 protected study days in every 5 week period

In recognition of the importance of medical education for Registrars, they
shall be entitled to medical education leave in accordance with Clause 26 of
the Collective Employment Agreement.

Attendance and participation in:
 Anaesthetic/ICU M&M meeting Wednesday and Friday 07300830
 Anaesthetic /ICU registrar tutorials and open presentations
 Anaesthetic/ICU audit days
 Grand Round - Friday 1245-1330 hours
 Radiology meetings – 12.45-13.30 every second and 4th
Tuesday of the month

ASSESSMENT and PERFORMANCE APPRAISAL
Ongoing in-training assessment and performance appraisal of registrar as required
by the College will be made by specialist staff and the Supervisor of Training, who
will provide a written report to the registrar at the end of each six monthly period.
This report shall be confidential to the trainee, Supervisor of Training and the Clinical
Head of department and will be available for inspection by other parties only with the
written approval of the registrar.
Formal case based performance evaluation will be taken from time to time for cases
on elective lists shared with a consultant. The results of these assessments will be
evaluated as part of each six monthly review.
The Clinical Head of Department and other senior medical staff will conduct a six
monthly review of performance of the registrar on behalf of Northland District Health
Board. This is usually delegated to the Supervisor of Training.

RESPONSIBILITIES FOR TRAINING and PROFESSIONAL
DEVELOPMENT OF OTHER STAFF.
Registrar will participate in education of other staff as determined by the Intensive
Care Consultants:
Teaching of House Officers/Registrars
Teaching anaesthetic technicians
Teaching nursing staff
Running the Morbidity/Mortality meetings each month
Running the Trauma and Transport Audits
This may consist of teaching during the management of cases in ICU as well as by
giving the occasional tutorial or lecture.

RESEARCH and REVIEW ACTIVITIES
Research - Participation in and evaluation of research is considered an important
aspect of Registrar training and duties.
Review - Registrar will participate in clinical audit and quality assurance activities,
including attendance at ICU, M&M and Surgical Audit Meetings.

OTHER REGISTRAR AND SPECIALIST COVER
Registrars will be required to cover absence of other senior and junior staff on the
same roster in certain circumstances, viz.
Cover for absence on annual/sick/conference/exam/course leave.
Cover for Specialist will be limited to cover of elective lists, providing that registrars
will not be called upon to perform duties beyond their experience and the proportion
of unsupervised lists will not exceed that recommended by the College.

HOURS OF WORK
Medical cover in the ICU is provided by rotating shifts over 24 hours/7 days.
Weekdays are covered by day (0800-1700); evening (1600-0030) and night (00000830) shifts. Weekends are covered by day (0800-2030) and night (2000-0830)
shifts. The RMOs are also rostered for periods of on-call contiguous with day or
evening shifts and may be required to undertake appropriate air ambulance
activities.
The service, in liaison with the RMO Unit, is responsible for the preparation of all
rosters.

In accordance with clause 8.1.2 this will be paid as a C category run
