Catch Basin/Culvert Cleaning Service
The Town of Hermon is requesting written proposals for catch basin and culvert cleaning
services. Bidders are requested to fill out and return the attached form to the Hermon Town
Office by 4:00 PM, Friday, October 30, 2015 at 333 Billings Road, Hermon, ME., 04401 in
an envelope marked Catch Basin/Culvert Cleaning Bid 2016-2018. The Town of Hermon
reserves the right to accept or reject any and/or all bids. Services are requested as needed for both
emergencies and general maintenance.
The following is a list of catch basins to include annually for vacuuming service:
Blackstream Road- 5 catch basins
Newburgh Road- 6 catch basins
Bog road- 1 catch basin
Hermon Rec Area at Elementary School- 2 basins

Town of Hermon may purchase additional services or change the list if necessary.

Bidder must provide catch basin vacuuming cost per basin for additional work not
already listed on bid form

Bidder must include culvert cleaning price per unit on bid form

NOTE: This is a three year contract. Award based on the total cost for three years.

By submitting and signing this bid, the bidder understands the Town of Hermon may accept or reject
any and all bids. Bidders are also required to file proof of general liability insurance in the amount of at
least $1,000,000 naming the Town of Hermon as "additionally insured". Bidders must also include proof
of workers compensation insurance in the amount of the statutory lower limits prior to the awarding of a
contract.
The Town of Hermon reserves the right to offer a contract extension with Council approval.
