To join the practice, all that is required is the completion of a straight forward joining form - you do not need to inform your current GP that you are leaving - your notes will be transferred straight to our practice Once you have registered you will be offered a comprehensive new patient medical check. The practice catchment area is A183 (Chester Road) to the North, B1405 (Queen Alexandra Road) to the East, Farringdon and Plains Farm (Silksworth Lane – Warden Lane – City Way B1286) to the South and the A19 to the West APPOINTMENTS We operate an appointment system. Appointments can be made either for a specific GP or for the next available appointment - please telephone or call into the surgery to make an appointment. The appointment book for the week ahead opens on a Friday morning. Please remember to cancel all unwanted appointments. URGENT APPOINTMENTS The practice has a number of urgent appointments each day for the Dr and the Nurse Practitioner. (Urgent appointments are not for Repeat Prescriptions, fitness for work certificates or for signing forms). NURSE PRACTITIONER We have a Nurse Practitioner available every day, who can deal with many of the problems that a GP would normally see. The Nurse Practitioner triages requests and will put you in with the most appropriate clinician. HOME VISITS Home visits should only be requested for those who are unable to come to the Surgery because of serious illness and infirmity. They should be requested before 10.30 if at all possible. Whenever possible we prefer to see you at the Surgery; if you do not feel well enough to sit in the waiting room we can make alternative arrangements. OUT OF HOURS PRESCRIPTIONS Patients may also attend the Grindon Primary Care Centre which is open 24 hours a day for minor injuries and illnesses - Telephone 0191 5195800 or Bunny Hill. 0191 5195800 (not 24 hours). If you are on repeat medication, please use the reordering slip attached to your repeat prescription. Remember to tick the boxes for the items required. Requests for medication can be submitted: · Over the phone · By hand to the reception or at some local chemists · By Post (Include a SAE if you want us to post it back to you) · Dropped off either in our outside post-box or in our repeat script post box by reception · Faxed to us on 0191 5282626 Requests handed in before 9.30am, will be ready after 4.00pm the next working day. CHAPERONES All patients are entitled to have a chaperone present for any consultation. The emergency GP service is provided by Primecare on behalf of the practice and commissioned by the SCCG. TEST RESULTS A mutually convenient telephone appointment will be made for test results to be given for blood and x-ray results. CAR PARK The car park is available for patient use and is at your own risk. There are two disabled bays near the entrance to the practice. The practice accepts no liability for any losses or damage caused whilst parked on its premises. We advise that patients do not use the car park in icy conditions LEAFLETS You will find a wide variety of Practice Leaflets which will inform you of all the services that we provide for our patients. These include a Patient Charter Leaflet and Comments and Complaints leaflets. COMMENTS, COMPLAINTS and COMMITTEES Comments on any aspect of the practice are always welcome. If you have any suggestions or are unhappy about any aspect of our service, write them down and drop them in our suggestion box in the reception. The practice also has a patients’ committee who can be contacted through our suggestion box – details from reception. If you wish to make a formal complaint against us, we operate the NHS complaints procedure: any letters of complaint should be addressed to the Practice Manager.
