Welcome to
Somercotes Medical Centre
We began using the current premises in October 1988. Parking is available for a limited number
of cars. Access to the practice, clinical rooms and patient’s toilets facilities have been designed
to help those with physical disabilities. The lobby area just inside the front door may be used by
parents for pram and buggies, and there are baby changing facilities available.
As you would expect, we ask that there should be no smoking in any part of the building or
grounds of the surgery, that mobile phones are turned off, and no dogs will be admitted other
than guide dogs for the blind.
We hope to use our surgery website as a way of presenting all our patients with an up to date
resource for all information relating to our practice. We will keep it current with any news,
developments & details that are relevant to the practice & our patients.

If you are eligible for a influenza vaccination we are now booking appointments for our clinics in

Appointments
To make the best use of time, we run a full appointments system. Patients with urgent problems
however, will always be fitted in when necessary at the end of the morning sessions. You can
make an appointment either by telephoning or by calling in personally. Please make your request
in plenty of time whenever possible.
Remember that each appointment lasts for only 10 minutes. It is designed to discuss one
complaint, so if you have more than one issue to raise please book a double appointment with the
receptionist.
Out of Hours
Please remember requests for medical assistance outside normal surgery hours should be
reserved for urgent problems only. However, if you should need help during out of hours, please
telephone 111.
Test Results
Please telephone the surgery between 1.00pm and 4.00pm to obtain the results of your tests.
For patients on regular medication, please make written requests for repeat prescriptions.
Please allow a minimum of two working days for your prescription to be ready for collection.
You will find a re-order sheet attached to one side of your prescription making it easy for you to
tick the items that you need.
Patients should be aware that you may be asked to see either a Doctor or Nurse periodically for
routine medication checks, and this may delay the issue of your prescription. This is why we
would ask that you should not leave ordering your prescription until you have only a few days
supply of medication left.
You may order your prescription in any of the following ways:
Post – You may post your request to us. If you wish us to post your prescription back to you
please ensure you enclose a stamped addressed envelope. Please allow extra time for postal
services delays.
Fax – Our fax number is 01773 601704
Chemist Services – You inform the local Pharmacy that you need your prescription. They will
notify us, collect the prescription and make up your medication ready for you to collect directly
from them. Please ask you local chemist if they provide this service.
By Telephone – We have a telephone ordering facility for the use of patients who have severe
mobility problems or are housebound. Please call us on 01773 601702 between 1.00pm and
3.30pm every weekday.

By written request - For regular repeat medication there is an attatched sheet with the
prescription showing a list of your medication. To re-order an item, please hand the list in at
reception, post it into the collection box within the entrance hall of the surgery or post through
the letter box if the surgery is closed. Please allow 2 working days for the prescription to be
processed
Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed.

Catchment Area
SOMERCOTES MEDICAL CENTRE CATCHMENT AREA
In order to give patients the best possible quality services, practices have the discretion to decide
upon their chosen catchment area, taking into account their list size, practice workload and GP
availability.
The area must provide ease of access for patients to attend the surgery and for the GP to conduct
their visits when required. Many NHS Services are disadvantaged by how the County borders
affect them; Community Services such as District Nursing, Health Visiting and Mental Health
Services for example are disadvantaged by being restricted when working across County
borders. This obviously impacts upon our patients living over the Derbyshire/Nottinghamshire
border.
It is for all of these reasons that Somercotes Medical Centre decided to remove Selston, Golden
Valley, Jacksdale and Westwood from our catchment area many years ago. It was however,
decided that we would not ask our current patients living in these villages to transfer to another
practice, apart from when they moved home. We do not accept any new patients from Selston,
Golden Valley, Jacksdale and Westwood for any reason.
OUR CURRENT PRACTICE CATCHMENT AREA IS SOMERCOTES, ALFRETON,
SWANWICK, RIDDINGS, LEABROOKS, PYE BRIDGE, JUBILEE, IRONVILLE AND
SOUTH NORMANTON.
We would of course be very pleased to welcome back any patients who are returning to our
catchment area should you choose to do so.

Are you a Carer or are you cared for?
The definition of a Carer is someone who looks after a relative or friend, young or old, who is
disabled due to a physical or mental impairment or by age.
Many Carers do not even realise that they are Carers for many reasons. For example, they may
be looking after a disabled child in a normal family setting, or the Carer may be a child
themselves and looking after a parent or sibling, or you may even be a friend who checks in on
an elderly neighbour.....the scenarios of Carers are numerous and very diverse.
The Practice would like to know of all of the 'Carers' and 'Cared for' patients within its list. We
are campaigning to help as much as we can. If we know of carers we can adapt the surgery
systems to make life easier. This may be doing such things as making the repeat ordering of
prescriptions easier, or advising you that the Cared for patient is in need of a health check so that
you are able to support them. You may like to make some other suggestions of how we can help.
We will need permission from the Cared for patient to be able to help and so arranging this will
help both you and the Practice.
We have leaflets and Carers Packs available from reception, and a simple form/ letter of
authorisation that will help us to work with you and of course any information that is shared with

Thank you to everyone who participated in our Carers Survey at Somercotes. We have looked
at the responses and feedback, and working with the Derbyshire Carers Association charity we
have created a pioneering new scheme of in- surgery support for carers in the area.
We now have a Carer Support Worker from Derbyshire Carers Association who comes to the
surgery once monthly to run a clinic for Carers who would like to get some support or guidance
with their caring role. If you are feeling over-whelmed or even if your coping fine Lisa, our
support worker, can work with you to:
Access additional funding
Access respite care
Apply for blue badges
Provide one to one support
Create emergency plans in case you become unwell yourself
Get extra help in for you and your loved one
Lisa is happy to offer support in person at Somercotes Medical Centre, on the telephone or

Citizens Advice Clinics
Mid Derbyshire Citizens Advice Bureau provides a weekly advice session at the surgery on
Tuesdays between 2pm to 4pm on an appointment only basis.
This service offers free, independent, confidential, impartial advice and help in the following
areas:
• Welfare Benefits. Advising on entitlement and the claiming process, helping with the
completion of forms and advice and help with appeals.
• Money Advice. Assistance dealing with debt, including negotiation with creditors, drawing up
financial statements and advice on court procedures.
• Employment. Advice on employment rights, redundancy, dismissal and claims to an
Employment Tribunal.
• Housing. Advice and negotiation on tenancy rights, homelessness, and disrepair.
• Consumer problems. Advice on consumer credit, unsatisfactory goods and services, drafting
and writing letters and offering further negotiation.
• Relationships. Advice and information on issues such as divorce, separation, maintenance,
death and bereavement.
If you feel you could benefit from an appointment, please speak with one of our receptionists.
Home Visits
Home visits are only made if your illness prevents you coming to the Practice. Please consider
that a home visit takes more GP time than an appointment in the surgery, a GP is able to see 3 or
4 patient in surgery in the time it takes to complete one home visit.
If you are likely to need a home visit, please telephone the Surgery as early as possible,
preferably before 10.00am. .
It will help us if you could answer the questions asked by our reception team. but please note that
one of our Doctors may call you back to get a clearer picture of your problem.

Patient Registration
Registration with the practice
If you wish to register click on the link below to open the form. When you have completed all of
the details, bring it inot the surgery with two forms of ID as specified. When you visit the
surgery for the first time you will be asked to sign the form to confirm that the details are correct.
We ask you to fill out a medical questionnaire, this gives us background information that allows
up to care for you whilst we await the arrival of your medical records. There is an online version
of this file too, which you may fill out and send to us. When you come to the surgery you will be
asked to sign this form to confirm that the details are correct.

PRACTICE AREA
In addition to Somercotes, we accept patients from Alfreton, Riddings, Leabrooks Swanwick,
Ironville, Pye Bridge, Jubilee and South Normanton with the M1 marking the outer edge of our
practice catchment area. We are not able to accept registrations from outside of these areas as it
may impact on our ability to care for the patients living within our catchment area.
Change of Address or telephone number
Please let us know as soon as possible if your address or telephone number changes. It is
extremely important that we keep accurate records of address and telephone contact details.
Remember we may need to visit you if you are ill or contact you by telephone.

Test Results
Please telephone the surgery between 1.00pm and 4.00pm weekdays to obtain the results of your
tests. Remember that we are bound by confidentiality rules, and can only issue results to the
patient themselves or to Carers who have been previously authorised by the patient to receive
such information.
Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a
genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly,
and as amicably, as possible.
Simply contact the Practice Manager and she will set all the necessary wheels in motion. Further
written information is available on the complaints procedure from reception. We are continually
striving to improve our service. Any helpful suggestions would be much appreciated and a
suggestion box is located in the waiting area.
Travel Information
If you require any vaccinations relating to foreign travel you need to make an appointment with
the practice nurse to discuss your travel arrangements. This will include which countries and
areas within countries that you are visiting to determine what vaccinations are required. There is

It is important to make this initial appointment as early as possible - at least 6 weeks before you
travel - as a second appointment will be required with the practice nurse to actually receive the
vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second
appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and
above the normal prescription charge. This is because not all travel vaccinations are included in
the services provided by the NHS.
We are no longer a designated Yellow Fever Clinic.

GP Training
The Practice is a Teaching Practice. You may be offered an appointment to see one of our
Registrars. Please be aware that our Registrars are qualified Doctors who are learning how to be
our GPs of the future.
Medical students specializing in 'Obstetrics and Gynaecology', and 'Paediatrics' attend the
practice from time to time and may be sitting in on appointments with our own regular Doctors.
We hope that you will co-operate with us and help the students to learn about general practice.
However, you will be informed of their presence in advance, and if you do not want them to be
present at a consultation, your wishes will be respected. This will not affect your treatment in any
way.

Medical Records
Relevant information about all of our patients is held on the practice computer system. We are
registered under the Data Protection Act and as such all information held by the practice is
protected by the Act.
Patients should note that under the Freedom of Information Act, they are not only at liberty to
see information held about them by the Practice, but can have access to general practice
information.
All patient information is strictly confidential and your permission will be sought to release
relevant information to outside agencies, (such as insurance companies) before action is taken.
Carers:
As discussed in earlier sections, we do need to obtain patients' written permission before we can
disclose any information to Carers. If you have a Carer we will need to know about this and we
will ask for your written permission to authorise your Carer to act on your behalf whenever
necessary. This may be for such reasons as ordering repeat prescriptions, or obtaining test results
for you, or asking a Doctor to visit you if you become ill. Of course you can determine the level
of disclosure you wish to agree to.
Patient Care Data:
Extractions of GP data for care.data will begin in spring 2014. Practices will be notified by their
GP system supplier in advance of any planned data extraction and will be required to agree the
extract before it can be sent to the Health and Social Care Information Centre (HSCIC). Note this
data extract should not be confused with planned QOF extracts for the purposes of GP payments
in January.
Patients will now have received information about Patient Care Data Extraction. This will
have been received through a recent mail shot sent to patients.
Practices cannot opt out of extractions for care.data. Using its powers under the Health and
Social Care Act 2012, NHS England has directed the HSCIC to make this collection. These
Directions give the HSCIC the power to require the information and GPs are legally obliged to
comply with that requirement.
A copy of the leaflet is available at http://www.england.nhs.uk/ourwork/tsd/care-data/. Note that
the patient information line does not come into effect until 6 January 2014. From 6 January
until 31st March 2014, you can refer patients who have questions or queries in relation to this
leaflet to the patient information line. The number is 0300 456 3531 and is on the back of the
leaflet. It is important to note that if a patient objects to their confidential information being
shared then the GP practice will need to add the relevant objection codes into a patient's record.
The patient information line cannot register objections. The patient information line will only be
able to manage calls in relation to the content of the leaflet.
You can decide to opt-out of this Data Collection yourselves however. Please inform the surgery
if this is your decision.
Zero Tolerance
This Practice operates a ‘Zero Tolerance Policy’. This policy explains the action the Practice will
take in the event of acts of violence, abusive behaviour or aggression within the practice
premises.
Aggressive behaviour will not be tolerated at any level. The aggressor will be evicted from the
premises and will be reported to the police authorities which may result in prosecution.
An aggressive patient will be removed from the practice list. This policy is for the protection of
the Doctors, practice staff, patients and any visitor to the practice.
Practice Security
The Practice has now upgraded its security levels.
Our new car park is protected by the installation of security gates on the entrance and exits.
These gates will be open during surgery hours only. Please be aware that if your car remains
parked in our car park when the surgery closes, you will be unable to leave the car park until the
gates are re-opened for surgery either the following morning or on the Monday morning
following a weekend.
Our premises are also now protected by CCTV cameras which also have night vision.
PALS
PALS (Patient Advice and Liaison Service)
The NHS employs over a million staff in thousands of locations. It is a large and complex
organisation providing a broad range of services. It is not surprising that sometimes you or a
loved one may feel bewildered or concerned when using the NHS. And this can be at times when
you are feeling at your most vulnerable and anxious.
So, what should you do if you want on the spot help when using the health service? The NHS
expects all members of staff to listen and respond to you to the best of their ability. But
sometimes, you may wish to talk to someone employed especially to help you. The Patient
Advice and Liaison Service, known as PALS, has been introduced to ensure that the NHS listens
to patients, their relatives, carers and friends, and answers their questions and resolves their
concerns as quickly as possible.
PALS also helps the NHS to improve services by listening to what matters to patients and their
loved ones and making changes, when appropriate.
What does PALS do?
In particular, PALS will:







Provide you with information about the NHS and help you with any other health-related
enquiry
Help resolve concerns or problems when you are using the NHS
Provide information about the NHS complaints procedure and how to get independent
help if you decide you may want to make a complaint
Provide you with information and help introduce you to agencies and support groups
outside the NHS
Inform you about how you can get more involved in your own healthcare and the NHS
locally
Improve the NHS by listening to your concerns, suggestions and experiences and
ensuring that people who design and manage services are aware of the issues you raise
Provide an early warning system for NHS Trusts and monitoring bodies by identifying
problems or gaps in services and reporting them.

Dr Hambley joined Somercotes in 1990. He has a wide range of
medical interests and provides the surgeries minor surgery service.
He is very experienced in joint injections and is able to treat a wide
range of problems.
Dr Antony
Dowd
MB ChB MRCP MRCGP
Dr Dowd qualified as a doctor in 1987 and spent 10 years working as
a hospital doctor in general medicine and nephrology before
becoming a GP and joining the practice in 1999 as a 3/4 time GP.
He has a wide range of interests and can carry out various joint
injections. He has a particular interest in education and is a GP tutor
for medical students as well as being a GP trainer with the Derby
vocational training scheme.

Dr Pooley joined the practice in 2011. Although he has a wide range
of medical interests, he has specific interests relating to the
relationship between psychology and health, medical education,
emergency medicine, and sports and musculoskeletal medicine. He

Dr MacLachlan joined the surgery in 2013. She is interested in the
management of diabetes, gynaecology and contraception - she is
responsible for fitting contraceptive coils in the practice. She is also a
GP trainer for the Derby training scheme.

Dr. Heafield joined the practice in 2013, having worked and trained
in the local area for the past 8 years. She is interested in all areas of
medicine, particularly gynaecology, sexual health, diabetes and
healthcare of the elderly. In addition to working part time at the
practice she is also a member of the Derby Mountain Rescue Team

Dr Nicky Hambley has worked part time in the practice since 1990,
having completed her GP training in Nottingham. She is interested in
all areas of medicine, particularly paediatrics, gynaecology, and
adolescent health. She enjoys teaching, and for many years she has
been a GP tutor for medical students.She is a trustee for the local
charity Valley CIDS, which provides the BLEND youth centre in
Ripley.

Counsellor: Our Counsellor service is accessed by visiting a Doctor, who will
discuss if the Counsellor is the best way forward for you.
District Nurses: The District Nurses are based at Alfreton. The team provide the
nursing assistance required by patients who are housebound, or who have recently
been discharged from hospital and need follow-up care.
Health Visitors: The Health Visitor and Sure Start team are also based at Alfreton
but hold clinics locally and work closely with our GPs to support families with
children from birth to school age.
Midwife: Our Midwife offers a range of antenatal care, and postnatal Mother and
Newborn care. She also provides additional services to help patients progress
successfully through their pregnancy and the delivery of the baby.
Citizens Advice Counsellor: We hold weekly sessions, and appointments can be
made with our reception team.
