The Don O. Tootle Scholarship Application
For The 2015 – 2016 Academic Year
AWARDS AVAILABLE:
We’ll be awarding two (2) $1,000 scholarships for furthering education. One will be awarded through our MedPro division and one will be awarded through our Ohio HealthCare division.
ELIGIBILITY:
1.
Applicant must be a member of Ohio HealthCare Federal Credit Union (either through
MedPro or Ohio HealthCare) in good standing and remain so throughout the life of the scholarship.
2.
Applicant must be planning to attend an accredited academic institution during the 2015-2016 academic year.
3.
The applicant must be planning to enroll in a course of study at, and subsequently be accepted to, an accredited academic institution and able to prove that he/she has been accepted.
4.
The applicant must be registered for at least 6 credit hours per semester/quarter.
5.
Employees and Board members of Ohio HealthCare Federal Credit Union and their family members are not eligible.
SELECTION:
The scholarships will be awarded based on a combination of factors including, but not limited to; the essay, extracurricular activities, and community/philanthropic activities. The essay will be judged on content, neatness, spelling, and grammar.
REQUIREMENTS:
1. Application and essay must be completed and received no later than March 31, 2015 - no exceptions
can be made.

JUDGING:
A selection panel, made up of two Credit Union members and two Board members, will judge all applications.
The winners will be notified in May 2015.
