Health club written report to certify patient fit for exercise

MEDICAL EXAMINATIONS AND REPORTS
Work of this nature will be charged depending on which of the following scenarios are requested of
GP: Extract from Medical Records only
 Report on Proforma- usually with examination (for example used for all driving reports)
 Written Report (with exam) to provide detailed opinion and statement on condition of patient
The above work could include accident or sickness Insurance claims, Employment requests,
Private Medical Insurance paperwork, Fitness for Education or
Suitability assessments for pharmaceutical trials.
All fees will be applied on the basis above.
