The doctors and staff at the Henry Moore Clinic in Castleford are proud to offer the highest standard of patient-centred healthcare. We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

In the case of urgent need when the practice is closed you can call NHS Direct

Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens.

If your condition is non-urgent you can expect to see a GP within two working days, though you may have to wait longer to see a particular GP. If you don’t need and appointment within two working days you have the option to book up to 12 weeks in advance if this more convenient for you

Patients may speak to a GP during surgery hours but you may be asked to call back at the end of surgery.
A Triage Nurse will take your call if there is no GP available
This is a training practice for future GP’s. In addition to the above list of Doctors, there will usually be a Registrar in training with the practice for a period of 6-12 months.As part of the training requirement, a consultation may from time to time be recorded by video. This would only be with the patients consent.
If possible please try to telephone reception before 10am if you require a home visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health
Centre.

Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.
From 1 April 2009, the charges are:
Prescription (per item): £7.20
12-month prepayment certificate (PPC): £104.00
3-month PPC: £28.25
If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is £7.20, whereas a three month PPC will cost you £28.25 and a 12 month PPC £104.00.

Minor operations can be done at our Fryston Road Surgery. These operations are carried out by
Dr MJR Blackamore. If you require any minor surgical procedures the GP you are seeing will place you on the waiting list and you will be contacted by a member of the practice team.
Nurses based at our practice treat patients for a wide range of common conditions as well as specialist clinics so you may be offered a triage appointment with the nurse if no GP appointment is available.

Please call between 1.00pm and 3.00pm to enquire about your test results as our reception staff will have more time to deal with your request between these times.
Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the practice.
It is your responsibility to check your results and to make an appointment to discuss them with your doctor if your are advised to do so.

The doctors welcome new patients who live within our practice area. If you live in our practice area and would like to register with us, please complete one of our registration forms that are available on reception at both surgeries. You will be registering with the practice rather than an individual GP and are free to see any available GP when booking appointments.
As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. Your registration will be complete when you have had a new patient health check.

Identifiable information about you will be shared with others in the following circumstances:
To provide further medical treatment for you e.g. from district nurses and hospital services.
To help you get other services e.g. from the social work department. This requires your
consent.
When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and
Government plan services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us know.
Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.

We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against our staff or patients. If a patient is violent of abusive they will be warned to stop their behaviour either in person or in a letter. If they persist we will exercise our right to request their removal from our list if necessary.
