Welcome to C M K Medical Centre
We are a busy urban practice consisting of ten GPs, 1 Registrar, a nurse Practitioner and four
practice nurses with district nurses, health visitors and midwifery services. We work
closely together for the benefit of the patients.
We believe that we provide a choice of personal care with a wide range of medical interests. In
line with modern practices we use a comprehensive computer service which is now a key tool in
patient care, record keeping, prescribing and preventative medicine.
We are a training practice which involves fully qualified doctors (registrars) working with us to
become General Practitioners. Occasionally, we may also have medical students working
alongside our doctors and at such times you will be asked permission for these professionals to
be present during your consultation. You do, of course, have the right to refuse.
Vitamin D
Are you taking enough Vitamin D? Children under 5, pregnant women and breast feeding
mothers are encouraged to take Vitamin D supplements. Read about it in the following

When We Are Closed
In common with 90% of Milton Keynes doctors, our out of hours calls are dealt with by a cooperative of doctors – MKUCS.
Please telephone 01908 605775 to receive information regarding out of hours service.
You may be given advice, asked to visit the centre that are within the grounds of Milton Keynes
Hospital or be visited by the doctor on call.
NHS Direct
In the case of urgent need when the practice is closed you can call NHS Direct
(www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and
advice offered or arrangements made for you to see a doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.
Appointments
We operate an appointment system of 10 minute consultations with doctors and between 10 and
20 minutes with practice nurses.
Appointments can be booked up to 6-8 weeks in advance plus routine appointments are available
on the day.
Please ask reception regarding our On-line appointment bookings.
If you need to see a doctor urgently and all routine appointments are booked you may be offered
an emergency appointment. Please note that these are not for repeat medication, sick notes,
letters or routine problems.
Telephones are manned between 8.30 and 6.30 Monday to Friday.
A text reminder messaging services is available to all patients you will automatically be signed
up to this if you have given us a mobile number, if you do not wish to receive a reminder text
please speak to a receptionist.
It is important to cancel any appointments if they are no longer required. Please phone the
surgery or text a message to 07513 288933 please make sure that you quote your name, time and
date of appointment you wish to cancel (please note that this mobile number only accepts text
messages)
You can also cancel appointments via the on-line appointments service
Home Visits
The surgery with all its facilities, is the best place to hold a consultation, however, home visits
will be made if:


as a result of your illness, you are unable to be transported to the surgery.
the doctors consider that it would be medically unsafe for you to attend.
Requests for home visits should be made before 10.00am.
Repeat Prescriptions
Some patients who are receiving long term medication may be authorised by the doctors to have
a repeat prescription. The left hand sheet of the prescription is used by the chemist to dispense
the medication. The right hand sheet (request slip) is retained by the patient and used when
medication is next required.
To request a repeat prescription :


Send your request slip by post or bring it to the surgery indicating clearly which items
you require. If you supply a stamped addressed envelope your prescription will be posted
back to you. (Please allow extra time for posting)
Some Chemists provide a prescription collection service and will dispense your
medication ready for collection. Please arrange this directly with the pharmacy.


You can use our on-line appointment system to order medication, please contact the
surgery for a user name and password
Fax details of you request to 01908 295657 – please give all details as above
Some chemists will also deliver to your home.
Please give at least 2 working days notice for the prescription to be ready at reception and
longer if you require the pharmacy to collect it.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
From 1 April 2012, the charges are:



Prescription (per item): £7.65
12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for more than 3 prescription items in 3 months, or more than 13 items in
12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is
£7.65, whereas a three month PPC will cost you £29.10 and a 12 month PPC £104.00.



Minor Surgery
This is carried out by the doctor and assisted by the practice nurse.
This may include for example the removal or freezing of a wart or cyst, steroid injection for
painful bone conditions i.e. tennis elbow, frozen shoulder.
Contraception
Contraceptive services and advice, including contraception implant fitting service and
emergency contraception is available to all , please make an appointment with Ruth Tompsett
Nurse Practitioner.
For emergency contraception it is advisable to seek advice as soon as possible either at the
surgery during normal hours or after 6.30pm contact The Brook Clinic or MKUCS. These
services are entirely confidential and can be given either by your doctor or your local family
planning clinic.
Sexual Health
Sexually transmitted diseases are common and can affect any sexually active individual. If you
think you may have a sexually transmitted disease you can seek confidential advice from your
doctor or from Milton Keynes sexual health clinic: Tel No: 0300 303 8273.
HIV testing drop-in clinic
This is available at CMK Medical Centre every Wednesday between 15.00 - 18.00. There is also
a drop-in clinic on Mondays 13.00 - 16.00 at Fishermead Health Centre, Fishermead Boulevard
MK6 2LR.
Screening
Some diseases can be prevented or treated more successfully through early detection by
screening. In view of this we encourage our patients to attend for the following procedures:
Cervical Screening (Smear test)
A recall system exists to call eligible women between the ages of 25 and 64 years for regular
smear tests according to national protocols. Appointments are made with the nursing staff.
Routine Breast Screening
Arranged by the health authority, is available to all women between 50 and 70 years of age.
Patients are informed of their appointment by letter. A leaflet containing further details is
available t at the surgery. “Breast Awareness is encouraged in all women of all ages.
Bowel Cancer Screening
Bowel screening is available to all patients between the age of 60 - 69. You will be contacted by
letter
Testicular Self Examination
It is important that all men, particularly those aged 15-40, examine their testicles regularly and
they are aware of any changes, e.g. pain, swelling etc, that may be of concern. Information is
available at the surgery.

Test Results
Test Results
Please telephone for results of blood tests or x-rays between 10.00 – 3.00 whenever possible.
Blood tests and xrays take a minimum of 5 working days, although some tests do take longer.
Please note it is the patient’s responsibility to collect their own test results. Due to
confidentiality, results will not be give to a 3rd party unless consent has been given by the
patient.
If you would like to be notified by email regarding test results please contact the surgery. You
would only be contacted by email if you needed to be seen or collect a prescription
Health Visitors & Baby Clinic
The Health Visiters at the surgery run several clinics/sessions on a regular basis these are:
A baby clinic every Wednesday between 1.30pm - 3pm. This is a drop in clinic no appointment
is necassary.
An immunisation clinic is also held on a Wednesday between 12.30pm - 4.00pm appointment times are
sent for this clinic but drop ins are accepted if you are unable to make your alloted date/time.
A family drop in session is available on Thursday mornings from 10am - 11am.
A breast feeding group meets on a Thursday morning between 11.15am - 12.15pm
All childern under 5 are invited for regular developements checks.
Please contact the health visitors for any further information regarding the above, by phoning the surgery
between 9am - 10am or 4pm - 5pm. There is an answerphone for any messages.

questionnaire, following this you may be invited for a Health Check with the Nurse.

Confidentiality
The surgery holds manual and computerised records on all its patients.
This data is entirely secure and confidential and we are registered under the Data Protection Act
1998.
All staff who have access to medical records are obliged to observe strict rules of confidentiality
Under the Access to Medical records Act 1990, patients may view their records, although a small
charge will be made.
Records are used by the NHS for financial audit purposes and for anonomysed research to
monitor and improve health care.
If a patient wishes to be excluded from anonomysed research, then they should write to the
practice manager.
Exclusion from research will not affect your treatment.

Medical Records
Medical Records may on occasions be inspected by other doctors for the purposes of training and
education. You do, of course, have the right to object to this but we would ask that any objection
is put in writing to the Practice Manager.

Immunisation clinic at CMK is held every Wednesday between 12.30pm - 4.00pm (times may
vary during the holiday periods please contact the surgery)
Here's a checklist of the vaccines that are routinely offered to everyone in the UK for free on the
NHS, and the age at which you should ideally have them.
2 months:


Diphtheria, tetanus, pertussis (whooping cough), polio and Haemophilus influenzae type
b (Hib, a bacterial infection that can cause severe pneumonia or meningitis in young
children) given as a 5-in-1 single jab known as DTaP/IPV/Hib
Pneumococcal infection
3 months:


5-in-1, second dose (DTaP/IPV/Hib)
Meningitis C
4 months:

Seasonal Flu Vaccination
Influenza – flu – is a highly infectious and potentially serious illness caused by influenza viruses.
Each year the make-up of the seasonal flu vaccine is designed to protect against the influenza
viruses that the World Health Organization decide are most likely to be circulating in the coming
winter.
Regular immunisation (vaccination) is given free of charge to the following at-risk people, to
protect them from seasonal flu:






people aged 65 or over,
people with a serious medical condition
people living in a residential or nursing home
the main carers for an elderly or disabled person whose welfare may be at risk if the carer
becomes ill
healthcare or social care professionals directly involved in patient care, and
those who work in close contact with poultry, such as chickens.

Travel Information
If you require any vaccinations relating to foreign travel you need to make an appointment with
the practice nurse to discuss your travel arrangements. This will include which countries and
areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you
travel - as a second appointment will be required with the practice nurse to actually receive the
vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second
appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and
above the normal prescription charge. This is because not all travel vaccinations are included in
the services provided by the NHS.

Health Visitors
Specialist nurses are attached to the practice who are trained in child
development and health education to provide advice, information and
support to all families. Patients may contact them at the surgery. They
