Box 9 Lac du Flambeau, WI 54538 715-588-3324 1-800-472-7207 Tribal Benefits Assistance Specialists and Vocational Rehabilitation programs Public and private benefits includes but is not limited to items such as: Medicare, Medical Assistance, Social Security Disability Income (SSDI), Supplemental Security Income (SSI), Housing, Food Share, Health Insurance, Veterans’ Benefits, Consumer problems, Supportive Home Services Aging and Disability Resource Specialist (ADRS) New position available to Tribes. o Eligibility is dependent upon Family Care program available in County Tribe is located Serve: (Age 18 and over) o Generalist who serves as a liaison and customer advocate to tribal members o Provides basic information and assistance (referral), options counseling, and enrollment in publicly funded long term care services o Ensures culturally appropriate information about aging and disability resources and can effectively access long term care programs and services o Conducts information and assistance home visits for tribal members on behalf of the ADRC o Assist tribal members in preparing materials needed to determine financial eligibility Medicaid Eligibility Outreach Specialist (MA Benefits or Eligibility Specialist) Tribal Clinic or Health Center based position Funded through Wisconsin Medicaid Program (Contract administered by DHS Tribal Affairs) Serve: (All clinic or health center clients) o Identifying alternative resources for the Tribal clinic or health center to bill for services provided o Assist Tribal clinic or health center to enroll public or private insurance programs they are or may be eligible Tribal Disability Benefit Specialist (DBS) Two GLITC Shared Service positions for all eleven Tribes in the State of Wisconsin GLITC funded by a direct service contract from the State of Wisconsin through the Aging Disability Resource Centers (ADRC) initiative part of Family Care Serve: Tribal members age 18 to 59 living on or near a reservation Specialist who provide information and assistance with public and private benefit programs including application and appeal procedures for people with physical disabilities, developmental disabilities, mental illness and substance abuse Legal backup services for this position is provided by the Indian Law Office of Wisconsin Judicare Current as of November 27, 2012 Page - 2 - Tribal Benefits Assistance Specialists and Vocational Rehabilitation programs Tribal Elders Benefits Specialist (TBS or EBS) Aging unit based program focused on serving Tribal Elders Funded by the State, Federal Government, and Tribes and administered by the Area Agencies on Aging Services are coordinated with Aging and Disability Resource Centers (ADRC) Serve: Age 55 and over, except Ho-Chunk age 60 and over o Generalist for providing benefits counseling, assistance, and advocacy for public benefits for Tribal Elders Location of TBS o GLITC provides one position that is a shared services position for six tribes (Bad River, Lac du Flambeau, Lac Courte Oreilles, Red Cliff, Sokaogon, St. Croix) o Ho Chunk o Oneida o Menominee Tribe and County o Stockbridge Munsee Legal backup services for this position are provided by the Indian Law Office of Wisconsin Judicare Vocational Rehabilitation for Native Americans (VRNA) Vocational Rehabilitation for Native Americans develops and delivers vocational support to eligible Native Americans/Alaskan Natives with disabilities living in the defined service area that are sensitive to their individual cultural beliefs, strengths, resources, priorities, concerns and abilities so each may engage in gainful employment. Those eligible to participate in VRNA may receive support in the following ways: o Vocational counseling o Vocational evaluation and/or career exploration o Job seeking skills o Job development o Transition from High school to post-secondary education or immediate employment o Physical or technical aids needed to accommodate a disability in the workplace o Tools and equipment o Other Federally approved services needed to find, keep or advance at a job.
