This form must be used to initiate an Urgent Operating Advice when required by
GO/RT3350. When completed, it must be sent to the Network Rail National Operations
Centre for onward distribution to affected duty holders, and a copy to the head of
delivery, Traffic Operation & Management unit, at the Rail Safety and Standards Board.

Please distribute an Urgent Operating Advice in accordance with GO/RT3350.
1. The advice is required because of (please delete as applicable):
A member of staff has sustained serious injury.
2. State briefly the facts that support your request for an Urgent Operating Advice
under category (a) or (b) above.
At 2149 on Friday 23rd May 2008, an S&T team leader carrying out a
facing point lock (FPL) test was struck by a train and seriously injured.
The incident occurred at Kennington Junction between Radley and
Oxford while the team were working red zone with lookout protection.
The line speed is 90mph.
While the incident will be subject to a full investigation, the initial findings
suggest that the team leader and his assistant were working on the point
machine in the Down cess with the lookout positioned in the opposite
(Up) cess to increase his sighting distance round the adjacent curve.
The lookout gave a warning for a freight train approaching on the Up
line, which was acknowledged by the team leader and assistant, but
which then obscured the visibility of the lookout while it passed. At the
same time, a passenger train approached on the Down line, striking and
injuring the team leader.

All staff working on or near the line are reminded that:
1, If visibility reduces (e.g. falling darkness in the evening) while working
under red zone protection then you MUST stop work, move to a position
of safety and set up a more suitable safe system of work.
2, If contact with the lookout is lost whilst working under red zone
protection, then all staff MUST move immediately to a position of safety
until advised by the COSS that it is safe to resume work.
3, When warned of an approaching train, and you have moved to a
position of safety, you MUST remain vigilant for the approach of trains
on other lines.
3. What action has been taken to prevent recurrence and severe consequences?
All personnel who are required to go on or about the lines to take note of
the 3 items contained in section 2 above.
Further information will be issued as the investigation progresses.
4. State the justification for an Urgent Operating Advice, including the factors
considered in your assessment of the potential for recurrence and severe
consequences if it did recur.
This serious injury to a member of staff has potential for serious
consequences if there is a recurrence elsewhere.
