Instructions for Completion
Justification for the proposed numbers of animals to be used in research or teaching must be provided for all procedures
outlined in the Animal Use Protocol (AUP).
Please identify the number of groups, animals per group, number of times the procedure will be replicated and the total
number of animals used for each procedure and project listed in your AUP over three years. Add rows where necessary.
Justify the proposed group sizes using statistical estimates (preferable), experience from similar previous experiments,
or a literature review. In cases where statistical significance is not expected (e.g., breeding colonies, teaching labs, pilot
studies, antibody production, tissue harvesting, etc.), explain why the proposed numbers of animals are the minimum
necessary to achieve the desired outcome.
For Section D, please indicate how many and which procedures a single animal will go through. If applicable, please
identify the strain/genotype of the animals used.
Detailed instructions and examples of how this form should be completed can be found on the ACUC website.
Please submit this form along with your AUP to acuc@berkeley.edu.

Section C: Method Used to Determine Sample Size (check all that apply)
This is a pilot study, as similarly established studies do not exist. The proposed study will use a
small number of animals to determine the feasibility of a larger study.

Section D: Procedures Done on a Single Animal
Please indicate how many and which procedures a single animal will go through. If applicable, please
identify the strain/genotype/breed of animal that will be used in each procedure. Charts are highly
recommended for clarity.
