Dear A&E COSs,

Please refer to the poster for programme detail. 2 spaces will be allocated
to each A&E Dept. If you want to send a 3rd participant, fill in his/her name,
allocation of 3rd applicants into the course will be considered by their training
status and availability of spaces.

A course fee of $100- per participant will be levied, please send application
(one department one application form) with cheques to:
