NEURORADOLOGY FELLOWSHIP APPLICATION PROCEDURE
We must have the following materials in order for your application to be considered complete:
1. A completed application form and a personal statement addressed to Dr. Nancy Fischbein,
Neuroradiology Fellowship Director.
2. A current CV, which includes date of birth, place of birth, and citizenship.
3. Three letters of recommendation addressed to Dr. Fischbein: one from either the chairperson of your
department or the director of your residency program, and two from additional faculty members in
your department.
4. If you are a foreign medical graduate, copies of your ECFMG certificates Steps 1 & 2.
5. All application materials must be submitted via email by Friday, December 4, 2015.
6. Please note: You must have a valid California Medical license at the time of your Fellowship
commencement, beginning on July 1, 2017.
7. Note: For your second year of fellowship you must have a valid X-Ray operators license.
Your personal fellowship application file will be submitted to the Fellowship Director upon completion of all
required application materials.
