Community Grants to Address Health Disparities in the Gulf Region Request for Applicants Purpose: The Gulf Coast has been impacted by major natural disasters in the past few years. These disasters disrupted families and communities and impacted access to health care and the economic and mental health of citizens. This funding opportunity seeks proposals from community based organizations to address a health disparity issue(s) of significance in Gulf region. Funds for this project are supported by Department of Health and Human Services, Office of Minority Health, CFDA no 93.004, and Grant number MPCMP061011-01-07. Application Due: October 31, 2010 Notice of Award: November 5, 2010 Funding Cap: $ 20,000 Project period: November 15, 2010 to March 15, 2011 Eligibility: Community organization based in coastal Alabama, Mississippi, or Louisiana such as non-profit service providers, federally qualified health centers, civic organizations, churches, professional societies, and schools. Types of Project: community outreach, educational and preventive services, and augmentation of clinical services in partnership with primary health care provider Allowable Costs: educational materials, small devices such a glucose meters, personal computers, printers, contracted services ( must justify how the subcontractor related to the project, cost, contact information, MSM must have copies of all subcontracts) , general and laboratory office supplies.
