Clinical Networks and Streams Purpose:
To link groups of health professionals from primary, secondary and tertiary care to work together in a
coordinated manner
To build staff capacity and improve service delivery to ensure equitable provision of high quality, clinically
effective care.

Facilitate strong relationships to encourage district wide innovation
Enable clinical practice innovation to minimise clinical variation and waste whilst maintaining clinical standards.

supportive care and end of life / palliative care
Develop area wide evidence based dialysis vascular
access management processes to plan, map,
manage and educate patients and staff. This
includes:
 alignment of staffing resources
 development of clinical pathways and education
materials.
Investigate and Develop plan to:
 increase people dialyzing at home
 reduce patients with known CKD using
temporary access device at first dialysis
 Support all staff to complete online respecting
the difference package
 Identify and prioritise staff and units to attend
cultural respect training
 Undertake review of all units for cultural safety
 Look for opportunities to participate in
Aboriginal events such as NAIDOC week
