No: Date of Issue: Issue: Page: Renal Dialysis Unit NP/RD/J 004 11.01.2012 5 1 of 4 JOB DESCRIPTION TITLE: Haemodialysis Patient Liaison Officer RESPONSIBLE FOR: See Job Summary ACCOUNTABLE TO: Renal Unit Head Nurse & Head of Nephrology & Dialysis JOB DESCRIPTION: 1.0 JOB SUMMARY: The Haemodialysis Patient Liaison Officer will act within the multidisciplinary team, liaising between nursing staff, the medical team and patients, collating relevant patient information which will then be utilised to assist the medical staff in the process of the monthly review of all regular haemodialysis patients. He/She will also ensure that any changes arising from the review will be discussed with the patients and will be reported to the Nurse in Charge of the Dialysis Unit allowing open lines of communication to be maintained within the department. She/He will be responsible for translating verbal and written forms of communication from Arabic to English and vice versa as required by staff and patients. 2.0 DUTIES AND RESPONSIBILITIES: 2.1 Monthly Review of Patients: 2.1.1 He/She will be accountable to the renal unit Head Nurse and will liaise with the Head Nurse regularly as required to discuss issues raised by the ongoing monthly review of haemodialysis patients. 2.1.2 He/She will be responsible for reviewing the patient’s dialysis file on a monthly basis and making note of the comments written by the named nurses on SUR/RD 031 (named nurse documentation sheet). 2.1.3 He/She will discuss the patients monthly progress with the Nurse in Charge of RDU and report findings to the Nephrologists at the time of monthly review. 2.1.4 He/She will inform the Nurse in Charge of RDU of any specific laboratory tests that have been requested by the Nephrologists for particular patients for that month (outside of the regular tests required). 2.1.5 He/She will be responsible for assisting the Nephrologists with the monthly review of all haemodialysis patients’ medications, blood results and problems. The Nephrologists will then review the patient, making any necessary changes to the prescription and will sign the prescription accordingly. The Patient Liaison Officer will not handwrite any item on the computerized prescription. 2.1.7 He/She will receive the patient’s monthly medication from Pharmacy and will ensure that any changes made to the patients’ prescriptions by the Nephrologists is explained to the patients and that their understanding is checked before the medication is distributed. 2.1.8 He/She will report all changes made by the Nephrologists (of patients’ treatment regimes, dialysis prescription, medications, diet or fluid restrictions) to the Nurse in Charge of RDU via verbal and written communication. 2.1.9 After discussion with the Nurse in Charge of RDU, the Patient Liaison Officer will then report any changes in treatment regime to the patients. 2.1.10 He/She will ensure that any issues or concerns raised by the patients are also reported Nurse in Charge of RDU to ensure sound communication is maintained within the multidisciplinary team. 2.2 Serology Status of Patients: 2.2.1 Nursing staff will report any changes in patients’ serology status to the Patient Liaison Officer, who will subsequently inform the Nephrologists and the Head Nurse for further action to be taken. 2.2.2 The Patient Liaison Officer will co-ordinate the swapping of dialysis spaces for those patients whose serology status has changed. He/She will also co-ordinate the swapping of dialysis spaces for those patients returning from vacation or from other units onto the third shift until serology results have returned and serology status has been determined. 2.2.3 He/She will also liaise with the department of Preventive Medicine to ensure that relatives of patients with positive serology results are followed up and vaccination programmes can be implemented. He/She will collate the patient questionnaires and analyze the findings appropriately for action to be taken by the physician/nursing staff. 2.2.6 He/She must promptly report complaints and incidences in accordance with hospital policy. 2.2.7 He/She will attend mandatory lectures, i.e. Fire & Safety according to hospital policy. 2.2.8 He/She will participate in Professional Development Programmes as necessary. 2.2.9 He/She will help the renal unit patient educator and renal unit social worker liaison officer whenever necessary. HOURS OF WORK: 3.1 48 hours per week for Registered Nurses 3.2 140 hours per month for MSD Technicians REQUIREMENTS: 4.1 At least five years experience as a Dialysis Technician/Renal Nurse 4.2 Current certificate of proficiency in the administration of medicines (If not RN). 4.3 Excellent communication skills. 4.4 Sound knowledge base of renal specific diet and fluid requirements. 4.5 Sound knowledge base of renal specific drug therapies. 4.6 Sound Knowledge base of renal replacement therapy.
