Dear Resident,
As part of the Community Pediatrics Training Initiative (CPTI), we are compiling stories
from residents that are doing exciting work with community partners in an effort to learn
from these experiences and share resident stories with a broader audience. Please
complete each of the following questions related to your community pediatrics
experience. Please try to limit your overall response to 1-2 pages. With your
permission, your experience may be developed into a narrative account and shared with
the Community Pediatrics Training Initiative network at-large through the CPTI monthly
newsletter and web site. Your story will also be considered for publication in the AAP
News, Pediatrics in Review, and inclusion in a compendium of case studies to be
distributed to residency programs. (If you would not like your story to be published or
distributed, please be sure to indicate “no” in the checkbox at the end of this document.)
