Clinical Postgraduate PA Program CHECKLIST for Submission of Application with Curriculum review form for Initial Administrative Review 8.2011 IMPORTANT REMINDER: Programs must be operational with at least one enrolled PA resident at the time of application. One paper copy of the Application and appendices (also send on a CD or stick drive) Application and appendices must be two sided and three-hole punched. Table of Contents must be included. Letter-size, lettered tab inserts (not post-it notes) are to be used for each Appendix and Attachment. Be sure that holes in divider tab sheets are aligned with holes in the application pages. Copy should be secured by rubber band(s) and NOT submitted in a binder. Application should NOT include college catalogs Remember to include the curriculum review composite and program response in Appendix E. The program should not make any additional copies for submission as indicated below until it has successfully completed the administrative review process. After the program has successfully completed an administrative review, it will be so informed and asked to contact the ARC-PA to be scheduled for a site visit. The program will be asked to submit a draft of the site visit schedule to the ARC-PA, using the ARC-PA schedule template as on the website. When scheduled for the site visit, the program must also submit the accreditation fee of $3,5001. Programs that cancel their visit within sixty days of their scheduled visit will be responsible for any expenses associated with the visit that have been incurred by the ARC-PA. Programs should review the information on the site carefully and contact the ARC-PA staff with any questions. Information Available on the Postgraduate Program section of the ARC-PA web site: Accreditation Standards Accreditation Process: general information about the application process, and application. Costs: information about the costs and fees associated with the process Site Visit Team: information on the purpose of the visit and role of the team. Site Visit Protocol: provides details about site visit ‘etiquette’ and the planning needed by the program director in advance of the visit, during and after the visit. Site Visit Schedule: provides an overview of an acceptable site visit schedule with a link to a PDF copy of a detailed visit schedule template. Required Documentation: lists the materials required for the appendices of the application as well as the materials that must be available at the program during the visit. Application and Forms: includes links to important documents for those planning for accreditation. Some of links displayed on the web pages are included in the application packet mailed to the program director and are provided on the web site merely for convenience. The program is strongly encouraged to review other sections of the web site, particularly the Accreditation Resource page (http://www.arc-pa.org/Acc_Programs/Acc_resources.htm). After the ARC-PA presents workshops and facilitates discussions at national PA education meetings, it posts handout materials in the Program Resources section. Much of this material is applicable to preparing for a site visit.
