JOB DESCRIPTION SECTION: PROJECT: JOB TITLE: RESPONSIBLE TO: LOCATION: PURPOSE OF JOB: Prevention Chéad Chéim - Aftercare Residential Project Project Worker Project Leader / Team Leader Chéad Chéim 494 North Circular Road To provide care and support for the young people resident in the project as they prepare for independent living. 1. To work within the framework of the overall objectives of Focus Ireland. 2. To work with and under the supervision of the Project Leader & Team Leader. 3. To provide keywork to a number of assigned young people. 4. To undertake an assessment of the young people’s personal, family, social, accommodation situation and needs. 5. To contribute to the development of the Aftercare plan, in conjunction with the Aftercare Worker, which will act as the young person’s support plan. 6. To advocate on behalf of residents for housing, social work, social welfare/ financial, education, training and legal rights. 7. To liaise with and make referrals to relevant outside agencies. 8. To provide individual support and keyworking to residents of the Project. 9. To assist young people in finding suitable move-on accommodation. 10. To facilitate group work sessions for residents of the Project when necessary. 11. To assist in maintaining a good standard of hygiene in the general upkeep of the Project (including resident accommodation). 12. To attend Project, Team, Divisional and Agency meetings as required. 13. To maintain accurate and up to date case records, client files and statistics. 14. To assist in the evaluation of the Aftercare Residential Project. 15. To implement the policies, procedures and standards of the Aftercare Residential Project and of Focus Ireland, as well as the National Leaving & Aftercare Policy. 16. To assist in the implementation of the settlement policies and strategies of Focus Ireland. 17. To work in close liaison with and co-operate with other Focus Ireland projects, and other appropriate services which the young person may be engaging with. 18. In consultation, to undertake other duties and responsibilities as may be assigned by the Project Leader, Team leader or Section Manager. 19. To provide assistance with the referral procedures of the project when requested. 20. To liaise with and inform the Project Leader & Team leader, on a regular basis, of issues arising from work with individual cases. 21. To engage in supervision sessions on a regular basis with the Project Leader / Team Leader 22. To engage in relevant training as identified by the Project Leader / Team Leader. 23. To undertake such other duties as might be reasonably assigned from time to time in consultation with the Project Leader, Team Leader or Section Manager. Essential Relevant work experience working with young people in care or marginalised young people. Degree in social care or related field. Experience of key-working clients or individual case-work. Experience of managing challenging behaviour. Desirable Good understanding of the issues affecting young people leaving care and in aftercare. Flexibility. Understanding of Child Protection Report writing skills A working knowledge and understanding of therapeutic crisis intervention model.
