Investigator-Initiated Research 2013 Study Activity and Outcomes Year-to-Date as of September 2013 Patient-Related Outcomes 1. The Predictive Value of Her2/neu in Patients with DCIS and Other High Risk Breast Lesions: A Case-Control Study. The overall goal of the investigator’s proposed study is to examine whether overexpression of Her2/neu is a predictor of clinical outcomes in patients with Ductal Carcinoma in situ (DCIS) and other high risk breast lesions including Lobular Carcinoma in situ (LCIS), Atypical Ductal Hyperplasia (ADH) and Atypical Lobular Hyperplasia (ALH). The research team is currently testing the hypothesis that Her2/neu overexpression in the above mentioned breast lesions may be a reliable predictor of local recurrence of carcinoma, or an indicator of future cancer in a patient who presents with an initial diagnosis of LCIS or an atypical lesion. Using tissue blocks from the ADH and ALH patient cohorts, they have validated diagnosis, and stained the tissue for Her2/neu expression. Subsequently, the team will analyze the data to examine the correlation of Her2/neu overexpression to future risk of developing cancerous lesions. PI: Judy Tjoe, MD; Co-I: Sanjay Kansra, PhD 2. Anti-Coagulant Pharmacogenetic Clinical Trial Simulations to Predict Improved Patient Outcomes The study has developed a normalized “anticoagulation” database containing more than 157,000 unique patients treated with warfarin. This database provides an invaluable resource for research on developing algorithms supporting health providers in determining safe warfarin dosing for individual patients more quickly. Determining the incidence and factors of cardiotoxicity in breast cancer patients treated with anthracycline and/or Trastuzumab-containing regimen at Aurora Health Care by developing in house natural language processing software The study has gathered 2011 year data and identified 17 patients (with post-LVEF lower than 50%) who fall into chemotherapy-induced-cardiotoxicity category as defined in the proposal. We have completed extensive review of these 17 patients’ physician notes (majority over 100 pages) obtained from patient charts to capture all the terms for the symptoms that are associated cardiotoxicity, which established the foundation for building the NLP tool. Based on these 17 notes from the identified cardiotoxicity cases from year 2011, we were able to build a proto-type NLP tool, which has the ability to identify, search, highlight all the terms we are interested and associated with cardiotoxicity. This tool has the following features: It is webbased and can be run by any devices, such as PCs, smart phones, and tablets that have a browser. Furthermore, this allows the UWM and Aurora researchers to work more efficiently during iterations to refine the search tool. It provides a predesigned search option based on domain knowledge provided by Aurora researchers. This allows a user to search for files without having to come up with words or phrases to characterize cardiac toxicity side effects related to breast cancer 1 treatments. It also provides a user input option that allows a more knowledgeable user to further explore the medical reports. It has negation detection that allows our search tool to produce more accurate results.
