NB Fetal Alcohol Spectrum Disorder (FASD) Center of Excellence Centre d’excellence sur l’ensemble des troubles causés pas l’alcoolisation foetale (ETCAF) au N-B COMMUNITY FORM Family Service Moncton is pleased to announce the opening of NB Fetal Alcohol Spectrum Disorder (FASD) Center of Excellence. The Center of Excellence is a provincial, bilingual program funded by the NB Department of Health. The mandate is to prevent, diagnose, and support families and professionals working with individuals affected by FASD. The Center of Excellence is accepting referrals from professionals or families with youth from birth to 18 years of age inclusively who may have been exposed prenatally to alcohol. In order to make a diagnosis of FASD the team will need to confirm prenatal alcohol exposure. Criteria for referral: ● Complete the referral form ● Referral age is from birth to 18 years old inclusively, at time of referral ● Signed consent by legal guardian or parents is required All referrals to be sent to: Family Service Moncton 22, Church St, Suite T410 Moncton, N-B E1C 0P7 Attn: Program Manager NB Fetal Alcohol Spectrum Disorder (FASD) Center of Excellence Or by fax 506-858-8315 If the referred child or youth meets the requirements for a diagnostic evaluation, a Community Coordinator from your region or the First Nations Liaison will contact the legal guardian or parent of the child. They will assist the family in completing the paperwork and will support the family before and after the diagnostic evaluation process. To diagnose an individual with FASD is a lengthy process which includes completing a developmental history form, confirming prenatal alcohol exposure, gathering medical, educational and family history, gathering any professional assessments that the child or youth may have completed and collecting information from the child’s school or daycare. Once the documentation is complete and evidence indicates FASD, the child or youth is placed on a list to be scheduled for assessments with each member of the multidisciplinary team. The diagnostic evaluation is completed by a professional multidisciplinary team that includes a pediatrician, a speech and language pathologist, a psychologist and an occupational therapist. The entire process of assessment and evaluation takes place in Moncton NB and may take a four day period. The child must be accompanied throughout the process by a parent or caregiver.
