JOB DESCRIPTION Position Title: Program: Service: Accountable to: Issued By: Date Issued: CERTIFIED DIABETES EDUCATOR Health Diabetes/Chronic Disease Diabetes/Chronic Disease Health Service Manager Date Revised: December 30, 2014 Classification: Health PURPOSE AND SUMMARY Under the direction of the Diabetes/Chronic Disease Health Service Manager, the Certified Diabetes Educator will be responsible for the delivery of diabetes public health and education to First Nations communities. This will include the delivery of monthly education sessions, the coordination of quarterly screening clinics, researching current trends and prevalence of diabetes, reviewing, identifying, and addressing service gaps with respect to diabetes intervention, development and/or purchase of educational materials, and facilitating access to secondary and tertiary care where necessary. The Certified Diabetes Educator will positively impact the lives of the community members of the First Nation, provide community members with the education needed to manage their health, provide current and research-based education to community members and serve the community by promoting positive health care changes. DUTIES AND RESPONSIBILITIES 1. Service Responsibilities Under the direction of the Diabetes/Chronic Disease Health Service Manager, the Certified Diabetes Educator is responsible for: a. Providing individualized diabetes self-management education/training (DSME/T) to individuals and groups and; Job Description – DIABETES HEALTH EDUCATOR i. Collect DSME/T program participant assessment data, in a collaborative and ongoing manner; ii. Collaboratively develop educational goals, learning objectives and a plan for educational content and teaching methods with DSME/T program participants; iii. Provide educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods; iv. Collaboratively develop an individualized follow-up with each program participant; v. Evaluate effectiveness of educational services provided by measuring attainment of learning objectives; vi. Conduct a follow-up assessment upon completion of DSME/T programs services, using outcome measures; vii. Document assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records; viii. Utilize a team approach to provide services and collaborates and communicates with team members when needed; ix. Identify when a program participant’s needs are outside the scope of the instructor’s practice and expertise, plus arrange for additional services to meet needs; b. developing a regional public health and education strategy for nine First Nation communities; c. implementing quarterly public health and education sessions in each First Nation community; d. facilitating the coordination of quarterly diabetic screening clinics for each of Dilico’s affiliated First Nation communities; e. identifying and initiating plans to address service gaps between existing diabetic health service practices and optimal standards of care; f. providing on-going reviews and up-dates of the diabetes health program policies and procedures; g. working in collaboration with the Community Health Nurses (CHN), Care Manager and the Community Health Representatives (CHR) in each of the Dilico affiliated First Nations to manage the delivery of a comprehensive diabetes health education strategy; h. nurturing, consistently and actively, a collaborative working relationship between the identified partners i. participating in the development of training materials; D:\106744232.doc Page 2 of 5 Job Description – DIABETES HEALTH EDUCATOR j. k. 2. contributing to, and participates in, a continuous quality improvement to measure DSME/T program and to identify and address opportunities for improvement; appraising his/her performances to identify areas of strength and areas for improvement and to develop a plan for improvement and growth; Other Responsibilities Under the direction of the Diabetes/Chronic Disease Health Service Manager, the Certified Diabetes Educator is responsible for: a. b. c. d. e. f. 3. Reporting on a regular basis to the Diabetes/Chronic Disease Health Service Manager on all matters pertaining to the Diabetes Program; Liaising with community based CHR’s , health and social staff; Completing and submitting monthly reports and routine correspondence; completing and submitting a prioritized work plan according to approved format; participating in the monthly review and update; Collaboratively meet and plan as a team player with Dilico District Unit’s team members. Ensuring the development, planning implementation, and on-going evaluation of a comprehensive proposal which reflects the overall principles of the Diabetes Initiative Strategy. ACCOUNTABILITY The Certified Diabetes Educator is accountable: a. b. c. as a professional requirement to obtain appropriate physician consultation according to the Expectations for Consultation with Physicians by Registered Nurses in the Extended Class (Primary Health Care Nurse Practitioners (1)); for maintaining clinical and leadership competence through continuing education; to the professional governing body (the College of Nurses of Ontario); QUALIFICATIONS 1. Education BScN degree with Diabetes Educator Certification required. Diabetes/Advanced foot care certification an asset. 2. is Work Experience Three (3) years experience in community based service delivery, knowledge and experience in the delivery of pubic health and education programs, previous experience in the delivery of diabetes education. 3. Skills/Abilities a. b. current skills and knowledge in diabetes, including anatomy and path-physiology of diabetes and its associated complications; proven ability to identify community health service needs; design, implement and evaluate health program policies and procedures; proven ability to consult D:\106744232.doc Page 4 of 5 Job Description – DIABETES HEALTH EDUCATOR c. d. e. f. g. h. i. j. k. productively with a variety of human service resources; proven accountability; in-depth knowledge about current diabetes treatment management; able to lead and effectively manage groups; ability to develop a collaborative, therapeutic alliance with individuals; excellent interpersonal, written and verbal communication skills, including proficiency in computer applications, especially Microsoft Office; problem-solving and leadership skills; ability to maintain confidentiality; ability to follow direction and work within the policies, procedures and the vision, mission and core values of Dilico Anishinabek Family Care; good knowledge of the Anishinabek culture and issues affecting Anishinabek children, families and communities in and around the district of Thunder Bay; ability to understand and/or speak an Anishinabek language. CONDITIONS OF EMPLOYMENT a. b. c. d. e. must be willing and able to work flexible hours, provide on-call service on a rotational basis; must be certified in CPR and Standard First Aid; if the candidate is a regulated health care provider he/she must provide evidence of registration with the appropriate regulatory body; he/she must possess be able to travel regularly, have a valid Ontario Driver’s License and have access to a personal vehicle; a criminal records search and vulnerable sector search is required upon hiring.
