Page 1 NHS Lanarkshire – Information Pack POSITION: Consultant Physician Diabetes/Endocrinology & Acute Medicine POST INFORMATION The Department Current Post This is a new Consultant post in Diabetes and Endocrinology and provides an opportunity to join an enthusiastic and dynamic multi-disciplinary specialty team (currently with 4 consultants in the specialty). Sub-specialty interests are encouraged as is clinical research within the department. This new post has a small regular commitment to Acute Medical Assessment (1 fixed session per week) in addition to specialty and general medical inpatient work. In rotation with colleagues you will participate in the General Medical on-call rota (currently 1 in 22) and Physician of the Week rota within the ERU, currently 2.4 weeks per annum. Diabetes and Endocrinology Monklands hospital offers good opportunity for a wealth of outpatient and inpatient specialty work and along with the other acute sites within NHS Lanarkshire (at Hairmyres and Wishaw General Hospital) all the appropriate co-specialty care is available for diabetes patients and the majority of endocrine patients. We are closely situated to Glasgow where tertiary endocrinology services are located. There are strong links to Endocrine colleagues in Glasgow with regular multi-disciplinary meetings e.g. pituitary MDT, thyroid pathology meeting, endocrine oncology, etc. Further there are regular West of Scotland group meetings in the early evening for both diabetes & endocrinology which is a good opportunity to mix with local specialty colleagues. We are fortunate at Monklands to have a designated out-patient area for diabetes and endocrine work, the David Matthews Diabetes Centre. The centre is located in the grounds of Monklands hospital. Within the centre there are 7 clinic rooms, 2 podiatry clinic rooms and podiatry administration office. There are two rooms used for clinical measurement and clinic phelobotomy supporting all clinics. The centre also houses the administration space for the diabetes specialist nurses, dietitians and research nurses. Further there are two further clinical rooms used for counseling / individual education or research patients. A valuable space for team work is the large education room used for group education sessions including DAFNE courses or pump start courses and multi-disciplinary team meetings. Finally there is shared office space for medical staff and secretarial staff within the centre. Monklands hospital provides all specialized specialty care for diabetes and general endocrine patients supporting primary care colleagues. As in other Scottish NHS board areas, the Sci-Diabetes system is used for all diabetes consultations permitting coordination of all aspects of clinical care and national adult of diabetes service delivery. The 4 current consultant staff work together to share the general and subPage 2 NHS Lanarkshire – Information Pack POSITION: Consultant Physician Diabetes/Endocrinology & Acute Medicine specialty work permitting flexibility for on-call commitments or leave whilst at the same time improving continuity of out-patient service. Currently each week there are review Type 1 and complex Type 2 diabetes clinics, new patient diabetes clinics, a multidisciplinary foot clinic and an insulin pump clinic. There is a fortnightly combined Diabetes Renal clinic and a Young Person Clinic which includes the Paediatric Transition clinic. There are weekly General Endocrine and quarterly Endocrine Pituitary clinics. The diabetes service includes a full multidisciplinary team of specialist nurses, dietitians and podiatry services. Structured education is delivered to diabetes patients DAFNE for Type 1 diabetes and EXPERT by community colleagues for Type 2 diabetes. There is a diabetes MCN within NHS Lanarkshire, Dr Sue Arnott (GP) being the MCN lead clinician and Dr Sandeep Thekkapat (Consultant Physician at Monklands) is our lead Diabetes / Endocrine clinician. NHS Lanarkshire has diabetes research nurses based in the David Matthews Diabetes centre and we are part of the Scottish Diabetes Research network currently participating in a number of multi-centered trials. There has been considerable expansion within the insulin pump service within the last 12 months and this along with a new pattern of working for all General Medical Physicians within the Medical Assessment Unit improving the quality and continuity of patient care has led to the opportunity to recruit a new consultant colleague. We are keen to attract an enthusiastic colleague committed to continuing to improve services for diabetes and endocrine patients as well as colleague skilled at delivering good general medical care for general medical in-patients. This post offers 2.5 out-patient clinic sessions as part of a 10 PA contract with additional appropriate clinical administration time. Further EPAs for specialty work can be discussed. It would be anticipated that a complex T2DM or complication clinic would be included within the weekly timetable for the successful applicant but balanced with a mix of T1DM work and endocrinology. All specialty colleagues work jointly in clinics and this provides flexibility on a weekly basis but also opportunity for the successful applicant to contribute to sub-specialty areas following agreement with colleagues and the clinical director through job planning. Acute Medicine and the Emergency Receiving Unit Monklands hospital admits just under 14,000 acute medical admissions per annum, with around half of these coming from A&E and the other half constituting GP referrals. Most acute medical admissions are initially transferred to a short-stay bed on the Emergency Receiving Unit (ERU). A smaller number of patients will be admitted directly from GPs and/ or A&E to a specialty bed (Coronary Care, Infectious Diseases, Renal Medicine, Haematology or Dermatology). Approximately 50% of patients are discharged within 48 hours, the majority from ERU. Patients requiring a Page 3 NHS Lanarkshire – Information Pack POSITION: Consultant Physician Diabetes/Endocrinology & Acute Medicine longer stay in hospital are allocated a hospital inpatient bed on a specialty ward that is determined, as far as possible, by the patient’s clinical needs. The rota arrangements for Acute/ General Medicine at Monklands are currently under review and have changed substantially in 2014, with a view to moving to a new model that is more closely aligned to the Standards that have been developed by the Society for Acute Medicine (SAM). Enhanced front-door consultant continuity will be an important component of this model. Acute Physicians and General Physicians, supported by Care of the Elderly consultants, will provide a comprehensive ‘front door’ service. Funding has been sought for a substantial investment in new Acute Physician posts to support this development. Critical Care There is excellent 5-bedded Intensive Care Unit at Monklands Hospital, run by Intensivists/ Anaesthetists, but with ongoing consultant physician input into the care of relevant patients. A 4-bedded medical HDU has also been established, which is co-located with the existing Monklands Coronary Care Unit (CCU). A Primary Coronary Intervention (PCI) service for Lanarkshire was established in October 2008, which means that patients with acute STelevation MI are transferred directly to the cardiac catheterization laboratory at Hairmyres Hospital rather than being admitted to CCU at Monklands. Page 5 NHS Lanarkshire – Information Pack POSITION: Consultant Physician Diabetes/Endocrinology & Acute Medicine DUTIES AND RESPONSIBILITIES Main Duties The Out-Patient duties for the post will primarily be at the David Matthews Diabetes Centre where 2.5 PAs of clinic work per week will be scheduled. Further EPAs for specialty work are available and candidates are welcome to discuss this further on application. Clinics include new and review Diabetes (including subspecialty) clinics, new and review Endocrine and monthly General Medical follow-up clinics. In-Patient specialty work will involve an equitable share with specialty colleagues of consultation work within the hospital. The General Medical In-Patient duties for the post will be on Ward 14 sharing 16 beds within this ward with other specialty consultant colleagues in rotation. Formal ward rounds are required to occur twice per week as a minimum with daily contact or board rounds with juniors on days without formal ward rounds. The post holder will also join an established group of physicians, including 3 Consultant Physicians in Acute Internal Medicine, in providing high quality clinical care for emergency medical admissions. You will have specific responsibilities to provide senior leadership in the Medical Assessment Unit on a specified clinical day, i.e. you will be responsible for a fixed session per week. The post holder will also be expected to participate fully in the clinical and educational supervision of junior medical staff. You will contribute to protocol development, educational activities and clinical governance and audit activity. The post holder will contribute to the General Medical On-call rota on an equitable basis with other colleagues and this includes Physician of the Week in rotation (1 in 22 currently). Programmed work out of hours and weekends is payable on a sessional basis and also an availability supplement is payable for on-call duties out-with scheduled work. Page 13 NHS Lanarkshire – Information Pack POSITION: Consultant Physician Diabetes/Endocrinology & Acute Medicine IMMIGRATION Overseas Doctors and Dentists Changes to Immigration Impact of Change Further Information For the purpose of immigration status, overseas doctors and dentists are those who, regardless of where they obtain their primary medical/dental qualification, are not nationals of the European Economic Area (EEA). A list of EEA countries is attached. On 7th March 2006 the Department of Health announced changes to the immigration rules for postgraduate doctors and dentists. The changes came into effect on 3rd April 2006 and mean that all doctors and dentists who wish to work in the UK from outside the EEA will need to meet the requirements of an employment category such as the work permit provisions. New applications for permit-free training visas will no longer be granted for doctors undertaking training posts Non-EEA doctors and dentists will still be able to come and train in the UK and are still eligible to apply, in open competition for training posts provided all the other eligibility criteria is met. However, training posts will now be considered employment for immigration purposes.
