I. Context
The rain drops have filled the sky for two days; at last they let the
warmth of the sun in this morning. Birds are chirping and singing as if
they are rushing us to take the inpatient dogs out for some sunshine.
After the rain, the sky looks bluer than usual; the inpatient dogs going out
for a walk are quite happy and seemingly recover from serious illness.
However, a beautiful day like this often means more outpatients or
accident cases. Dr. Lin has just finished an operation of a dog’s pyometra;
as soon as he is walking out of the surgery room, he receives a phone call
from the front desk informing him a new case. Then he saw a panic lady
holding a frightened dog walking in.

Are you Ms. Chen? This is your registration card. Let’s
weigh the dog first, shall we?
Chen:
Ok.
Lin:
It’s 13.2 kg. Let the dog lie on the examination table. Now
can you tell me, what had happened to this dog?
Chen:
It was hit by my car when it ran into the driveway in a sudden at
midnight yesterday! It cried for a few seconds, then it could not
stand up. Please help and save him!
Lin:
First, let me take its rectal temperature, pulse, and breathing
rate. Next I will examine its physical condition.
Chen:
Thanks, doctor.
Lin:
Well, Ms. Chen, what did you say his name again? Age?
Did you refer him from another hospital?
Chen:
Well, doctor, he is a stray dog, so I know nothing about its
background information, but I would like to name him NN.
Lin:
Ok, we will need to do some physical examination. By the
way, did you see him urinate? Did you let any doctor examine
him?

Not yet.
Lin:
The temperature is 39.3 degree Celsius, pulse 132, and breathing
rate 64.
Chen:
Yes.
Lin:
Pupillary light reflex is normal, oral mucous membrane is
pink,
capillary refilled time (CRT) is 1.5 seconds, lymph nodes are
normal in size. Well, there are a little scratch on caudal back.
Chen:
Is it serious?
Lin:
Well, breathing and heart sounds are normal. There is no deep
pain and proprioception on both hind legs. There is no
cutaneous reflex after lumbar.
Chen:
So, how is he now?
Lin:
Well, after several checks, so far NN has no immediate danger.
However, after the neurological examination, we suspect that it
might have broken back.
Chen:
My goodness, what should we do?
Lin:
Well, we should take him to the radiology for his vertebra. In
addition, we need to check his diaphragm and bladder to see if
they are ruptured or there is any internal bleeding, etc.
