Outline of programme:
This post is designed to offer senior IPTS Fellows a robust training in
Neonatology. The rotation will provide up to 24 months experience rotating
through placements in different neonatal units designed to provide a
comprehensive exposure to all aspects of neonatal training. The initial
placement (year 1 of training) will be in a level 3 neonatal unit attached to a
large maternity unit offering substantial experience in the management of
preterm infants. This will be in Wishaw General Hospital and will be followed
by a second year of training either remaining in Wishaw General Hospital or
rotating to the Southern General Hospital, depending on the trainng needs of
the individual. The Southern General neonatal service is associated with the
regional fetal medicine service and the regional paediatric and surgical
services and provides substantial exposure to the medical and surgical
management of infants with congenital abnormalities and complications of
prematurity.
The IPTS trainee will be expected to undertake NLS, and Child Protection
training during the first six-months of the programme.

Departmental Information:
Wishaw General Hospital (initial placement)
The Wishaw General Hospital is the largest district general hospital in
Scotland. The neonatal unit located at Wishaw General Hospital is equipped
with 29 neonatal cots of which 8 designated for intensive care, 10 high
dependency, 11 special care and 6 transitional care cots. Conventional and
oscillation ventilation, inhaled Nitric Oxide, total body cooling treatment are
offered on the neonatal unit.
Royal Hospital for Sick Children (Yorkhill) and Southern General
Hospitals
Glasgow Royal Hospital for Sick Children, (Yorkhill) is the largest paediatric
teaching hospital in Scotland. It provides care, not only for children resident
within Greater Glasgow, but is also a tertiary referral centre for children from
the West of Scotland and, in some sub-specialties, from the whole of
Scotland. All paediatric medical and surgical subspecialties are represented,
including general medical paediatrics, cardiology, neonatology, neurology,
nephrology,
respiratory
medicine,
endocrinology,
gastroenterology,

immunology and infectious diseases, dermatology, haematology/oncology,
audiology, ophthalmology, ENT surgery, orthopaedics and general paediatric
and neonatal surgery. The hospital provides the Scottish national ECMO
service, is the centre for all paediatric cardiac surgery and the majority of the
Paediatric Intensive care for Scotland. The Southern General Hospital is a
large adult hospital site about 4 miles (6km) from Yorkhill. A newly expanded
maternity unit opened in December 2009 delivering 5,500-6,000 infants per
year and incorporating the Fetal Medicine Unit. The children’s hospital is
currently being rebuilt on the Southern General site, and by early 2015 both
elements of the neonatal service will have merged into a single unit on that
site.
Activity / Workload
Wishaw General Hospital (initial placement)
The unit provides acute and long term specialist care to over 60 infants a year
who weighed less than 1000g at birth and well over 1000 intensive care days
per year. This level 3 neonatal unit provides supporting service to the regional
maternity service that has 86 maternity beds with an annual delivery rate of
over 5000 births and provision for consultant delivered high risk perinatal
service.
Royal Hospital for Sick Children (Yorkhill)

The new neonatal intensive care unit at the Southern General Hospital
provides care for preterm infants and high risk regional deliveries due to fetal
anomalies. This unit currently has 8 NICU cots but has the capacity to
increase to 34 NICU cots over the next few years. Additionally, neonatal
intensive care services will be provided in the NICU in Yorkhill. In early 2015
this aspect of the service will transfer into the Southern General NICU
following the rebuilding of the children’s hospital on the Southern General site.
Neonatal care at Yorkhill is part centrally funded by the National Services
Division with several national services provided on site. This includes
neonatal cardiac surgery, ECMO, neonatal airway surgery, Vein of Galen
treatment (UK/European service) and Congenital Diaphragmatic Hernia
service. The NICU (12 ITU cots) is jointly managed by the neonatal
consultants and the paediatric surgeons. Neonatal ECMO for Scotland and
the North of Ireland is based in the unit and a team of 3 surgeons work with
neonatologists to provide cover. Postnatal transfers for newborns with
congenital anomalies are also common. Over 200 newborn per year are
currently transferred to Yorkhill because of the specialist services offered on
site: one third are cardiology patients, one third are transfers of pregnancies

affected by a congenital anomaly and the remainder are newborns requiring
the involvement of the other paediatric specialists on site.
Clinical Training and Responsibilities
Wishaw General Hospital (initial placement)
This busy unit provides a wealth of experience in the management of infants
of all gestations from 24 weeks upwards. Training will be provided in the use
of a range of neonatal therapies including conventional and oscillation
ventilation, inhaled Nitric Oxide and total body cooling. The neonatal unit has
facility for Neonatal ultrasound scan with on site Paediatric radiology support
and is equipped with telemedicine link for Paediatric cardiology consultation,
teaching and training purposes. Trainees have senior cover throughout the
week and on-call provided by a team of consultant neonatologists. The
department is involved in several multi-centre research projects. The
undergraduate students from Edinburgh University undertake a Paediatric
rotation on the neonatal unit at Wishaw General Hospital.

The combined service will see many infants with a broad spectrum of
congenital anomalies delivered on the Southern General site to provide
immediate access to surgical and post-operative care. Neonatal trainees (and
consultants) will work across both aspects of the service thus providing
trainees with a comprehensive neonatal clinical training including: medical &
& surgical care of extremely premature infants, preoperative management of
congenital heart disease, care of infants with complex airways, diaphragmatic
hernias, abdominal wall defects and Vein of Galen aneurysms and
management of infants referred for consideration of ECMO therapy due to
failure of conventional respiratory management. There is a multidisciplinary
neurodevelopment clinic for the follow up of preterm and high risk infants. Our
developmental physiotherapist, our speech and language therapist and one of
the clinical psychologists attend this clinic and input from a dietician is available.
Non-Clinical Training and Responsibilities:
There are a number of formal teaching sessions and other learning
opportunities provided every week in each department. Other relevant
meetings and teaching sessions are held regularly in other departments
locally. Junior medical staff have access to library, computer, office and
changing facilities. There are full opportunities for clinical research and access
to laboratory facilities can be arranged as required. Trainees also have

access to other relevant paediatric subspecialty teaching opportunities within
the Yorkhill element of the rotation.
Particular Training Opportunities:
This rotation is specifically designed to offer trainees a comprehensive training
package in neonatology balancing exposure to medical and surgical problems
and problems of both term and preterm infants. It achieves this by offering a
number of placements in units that have a varied and complementary casemix. Trainees will have exposure to some therapies and interventions that are
only available in a very small number of U.K. centres. The rotation is also
designed to allow a degree of tailoring of placements to meet the needs of
individual trainees.
Rota arrangements:
Rotas are constantly being reviewed to maintain New Deal compliance
together with the new arrangements that will be required by Scottish Medical
Training (SMT). Rotas therefore, may be subject to change.

Study Leave
Study leave is available for up to 30 days per annum on the same conditions
as apply to the Paediatric ST trainees
Salary
Salary is paid on the Specialty Registrar salary scale. Starting point on this
scale will be dependent on previous experience but is likely to be at ST3 level.

DECLARATIONS/CONVICTIONS STATEMENT

Registration with the General Medical Council or General Dental Council imposes on doctors
and dentists the duty to provide a good standard of medical care and to behave

conviction, an order binding you over or a caution, in the UK or any other country.
Applications for posts in the NHS are exempt from the Rehabilitation of Offenders Act 1974.
Application forms will include a declaration for applicants to complete declaring any previous
or pending prosecutions or convictions, including those considered “spent” under the Act.
Forms will also include a declaration of any cautions or bind overs.
We also need to establish if you have been the subject of any fitness to practise proceedings
in the past, or if any fitness to practise proceedings are being contemplated, by a licensing or
regulatory body in the UK or another country and this is also reflected in the declaration.
This information will be treated in confidence and will not debar you from appointment unless
the selection panel considers that it renders you unsuitable for appointment. In reaching
such a decision we will consider the nature of the conviction/action, how long ago it took
place and any other factors which may be relevant
Failure to disclose a criminal offence, having been bound over or cautioned or that you are
currently the subject of criminal proceedings, which might lead to a conviction, an order
binding you over a caution, or fitness to practice proceedings undertaken or being
undertaken by an appropriate licensing or regulatory body, may disqualify you from
appointment, or result in summary dismissal/disciplinary action and referral to the General
Medical Council (General Dental Council) for consideration if such a discrepancy came to
light.

For the purpose of immigration status, overseas doctors and dentists are
those who, regardless of where they obtain their primary medical/dental
qualification, are not nationals of the European Economic Area (EEA). A
list of EEA countries is attached.
On 7th March 2006 the Department of Health announced changes to the
immigration rules for postgraduate doctors and dentists. The changes
came into effect on 3rd April 2006 and mean that all doctors and dentists
who wish to work in the UK from outside the EEA will need to meet the
requirements of an employment category such as the work permit
provisions. New applications for permit-free training visas will no longer
be granted for doctors undertaking training posts
Non-EEA doctors and dentists will still be able to come and train in the
UK and are still eligible to apply, in open competition for training posts
provided all the other eligibility criteria is met. However, training posts
will now be considered employment for immigration purposes. Work
permits will only be issued where there is a specific post for the
overseas doctors that cannot be filled by someone from the EEA.
The changes in immigration requirements for trainee doctors and
dentists are a result of a Scottish Executive Health Department
announcement and immigration issues are the remit of the Home Office.
