NHS Lanarkshire – Information Pack POSITION: Consultant Neonatologist POST INFORMATION The This post will be based in the Women’s services directorate of NHS Department Lanarkshire The Neonatal Service provides regional level III neonatal medical care for a neonatal population of 6,500 per annum with over 5,000 births per year at Wishaw District General Hospital. It is part of the bed bureau/ Perinatal advisory service and accepts in-utero and ex-utero transfers from other maternity/neonatal units in Scotland and the rest of the UK. The neonatal unit is a member of the Vermont Oxford network, the West of Scotland Perinatal advisory service and part of the West of Scotland managed Clinical Network. The Neonatal Unit is made up of 8 intensive care, 10 high dependency, 11 special care and 6 transitional care cots. The unit provides over 1200 intensive care days per year. 8.2 WTE Consultants (including the above posts) currently provide separate daytime and out of hours neonatal cover in a hybrid format with both resident and non resident duties. Conventional and oscillatory ventilation is provided; other facilities include Nitric Oxide therapy, total body cooling and cerebral function monitoring. Full medical intensive care is provided to extremely low birth weight, very preterm infants and ill term babies. All infants of viable gestational age are accepted to the Neonatal Intensive Care Unit that is centralized at Wishaw General Hospital. The population has a high pathology rate as a result of the social deprivation of the area. Neonates requiring cardiac and surgical intervention are transferred to the Royal Hospital for Sick Children, Glasgow following stabilization. The Maternity Service that is also centralized at Wishaw General hospital has 86 maternity beds with a capacity for 5500 deliveries per annum. It provides a Consultant delivered high-risk peri-natal service offering a full tertiary level anomaly scanning service to selected cases. Routine antenatal USS with detailed foetal scan at 20 weeks gestation is offered to all pregnant women in Lanarkshire. There are 2 weekly high-risk pregnancy liaison meetings and once in 3 monthly Perinatal Morbidity /Mortality meetings. NHS Lanarkshire – Information Pack POSITION: Consultant Neonatologist Some full time Neonatologists provide outpatient follow-up, and there are designated clinics for children with Bronchopulmonary Dysplasia, Neonatal Abstinence Syndrome and a high-risk multi-disciplinary Neurodevelopment clinic. This post may have a designated Neonatal follow up clinic if desired. The service has designated therapists, including dietetics, physiotherapy, occupational therapist, speech & language therapist and Neonatal pharmacist as well as a counselor for psychology and bereavement support. The Neonatal Unit is within the catchment of the "West of Scotland Neonatal Transport Service" and has a Neonatal Community Liaison Service to support early discharge. The neonatal service has a designated breast feeding support midwife and discharge planning midwife to facilitate discharge from the neonatal unit. There are comprehensive laboratory support services at Wishaw General and the Royal Hospital for Sick Children, Yorkhill. Excellent communication/referral routes exist between the Neonatal Unit and Consultant colleagues in subspecialties of Neurology, Cardiology, Radiology, Neonatal Surgery, Genetics, Ophthalmology etc, locally and at the Royal Hospital for Sick Children, Yorkhill. The Neonatal Service actively participates in individual and multi-centre research projects. There is a weekly grand ward round; teaching programs aimed at junior medical staff; journal club and regular audit activities. We have an active Advanced Neonatal Nurse Practitioner (ANNP) program with qualified clinicians participating in the first and second tier rosters. The department’s resource persons are involved in newborn examination and newborn resuscitation training and assessment. Consultants in the department are involved as college examiners for the MRCPCH, DCH and START assessments. The neonatal service is situated adjacent to the Paediatric Unit consisting of a 40-bedded area. This is divided into a 16-bedded short stay, day-case triage area and a 24-bedded in-patient area. The successful applicant will have admitting rights to paediatric beds as required. There are 14 general paediatric and subspecialty clinics weekly. Sub-specialty services provided include diabetes, endocrinology, neurology/neurodisability, rheumatology and respiratory. Up to 2 supplemental PA’s may be available for Neonatal transport with the West of Scotland transport service. The post-holder will be expected to be resident on call on average one night a week, on a tier 2 roster that runs on a 5 week cycle. The successful applicant will join the existing team of 7 Consultants who work in the Neonatal service and 12 consultants in acute and community Paediatrics. You will have proportionate duties in the Consultant weekly attending rota for NICU, HDU, SCBU and Transitional Care and a 1:10 on call for weekends and weekdays. All consultant neonatologists with or without resident duties share between them full responsibilities for the babies admitted to the Neonatal Unit. We run an attending system with each baby having a named consultant Neonatologist to ensure a clear line of communication for babies under their care. The named consultant has responsibilities for decisions involving complex ethical and clinical management issues. The main duties and responsibilities of the post will include: Contribution to the daytime weekly attending rota for NICU, HDU, SCBU and Transitional Care, during which time other clinical duties will be cancelled. Day to day decisions about the clinical management of babies in the unit during their attending week Out of hours responsibilities as outlined above for resident night cover as applicable). The out of hours cover 9pm until 9.30am is resident, with 1 junior doctor /ANNP working as first tier grade. There will be another consultant as second non resident on call duty. Effort will be made (as far as possible) to have experienced ANNPs on the shifts when the Consultant is resident. The post contributes to the weekend attending/ non resident on call consultant cover Provision of cover for Consultant colleagues during periods of leave. NHS Lanarkshire – Information Pack POSITION: Consultant Neonatologist IMMIGRATION Overseas Doctors and Dentists Changes to Immigration Impact of Change Further Information For the purpose of immigration status, overseas doctors and dentists are those who, regardless of where they obtain their primary medical/dental qualification, are not nationals of the European Economic Area (EEA). A list of EEA countries is attached. On 7th March 2006 the Department of Health announced changes to the immigration rules for postgraduate doctors and dentists. The changes came into effect on 3rd April 2006 and mean that all doctors and dentists who wish to work in the UK from outside the EEA will need to meet the requirements of an employment category such as the work permit provisions. New applications for permit-free training visas will no longer be granted for doctors undertaking training posts Non-EEA doctors and dentists will still be able to come and train in the UK and are still eligible to apply, in open competition for training posts provided all the other eligibility criteria is met. However, training posts will now be considered employment for immigration purposes. Work permits will only be issued where there is a specific post for the overseas doctors that cannot be filled by someone from the EEA.
