Base:
NHS Lothian is seeking to appoint a full-time Consultant in General Adult Psychiatry
to join a dynamic, enthusiastic and forward thinking team.
The Mental Health Services in West Lothian are integral to the Lothian Mental Health
and Wellbeing Strategy. Inpatient Mental Illness services, including an Intensive
Psychiatric Care Unit, and Care of the Elderly with Mental Disorders beds are in St
John’s Hospital, Livingston. There are also two regional inpatient services covering
the South East of Scotland, a Perinatal Psychiatry Mother and Baby unit and an
Eating Disorders Ward. There is a wide range of out-patient clinics, both hospital
and primary care based.
This is an excellent opportunity to join a first class team in an active and progressive
teaching hospital environment based in Edinburgh and the Lothian’s.

NHS Lothian is an integrated NHS Board in Scotland providing primary, community,
mental health and hospital services. Mr Tim Davison is the Chief Executive and Dr
David Farquharson is the Medical Director.
The NHS Board determines strategy, allocates resources and provides governance
across the health system. Services are delivered by Lothian University Hospitals
Services (LUHS), the Royal Edinburgh Hospital and associated mental health
services, 4 community health (and social care partnerships (CH(C)Ps) in Edinburgh,
West Lothian, East Lothian and Midlothian, and a Public Health directorate.
NHS Lothian provides services for the second largest residential population in
Scotland – circa 800,000 people. We employ approximately 24,000 staff.

Main Duties and Responsibilities
This is full time position of a Consultant in General Adult Psychiatry (10 sessions).
This is a chance to be part of a well-established team at a time of positive
development. The post holder will deliver care and promote a multi-disciplinary and
multi-agency approach to a catchment area defined by a number of local general
practices.
Clinical
 To work jointly with consultant colleagues to lead and shape the provision of
Psychiatry Services within West Lothian
 To promote a multi-disciplinary/multi-agency approach
 To be responsible clinically for the provision of inpatient and outpatient care to
a population of patients defined by their registration with a group of local
general practices. This will include planned and unscheduled care and
consultant responsibility for relevant patients under the care of the Acute Care
and Support Team (ACAST).
 To participate in the consultant rotas for day time duty consultant and out of
hours on-call.
 To cover for colleagues annual, study and short term sick leave.
 To be an Approved Medical Practitioner in terms of the Mental Health (Care
and Treatment) (Scotland) Act 2003 and the Adults with Incapacity (Scotland)
Act 2000 and undertake the required duties in relation to this
 To provide psychiatric court reports as requested by the local legal authorities
and reports in conjunction with aspects of incapacity legislation
Location
 It is anticipated the principal base of work will be St John’s Hospital but as
part of your role you may be required to work at any of NHS Lothian’s sites.
On-call
 The post holder will be required to participate in the psychiatric consultant oncall rota for the West Lothian psychiatric service out of hours, on a 1:9 rota,
covering old age psychiatry, mother and baby unit, eating disorder unit and
general psychiatry.
CPD


NHS Lothian is committed to provide Consultants with time and resources for
their Continuing Professional Development (CPD) requirements. There is a
local establish CPD peer group for West Lothian Consultants, which meets
monthly, in addition to weekly journal clubs during term time.

Revalidation Performance Management
 Comply with revalidation and appraisal process. Revalidation commenced on
3rd December 2012 and is the process by which licensed doctors are required
to demonstrate on a regular basiis that they are up to date and fit to practice.
Licensed doctors have to revalidate, usually every 5 years, by having regular
appraisals with their employer. You are required to ensure:
o You make NHS Lothian your Designated Body using the GMC website
o Know your revalidation date
o Attend regular appraisals
o Enroll in the Scottish Online Appraisal Resource (SOAR)
Medical Staff Management
 To work with colleagues to ensure junior doctors’ hours are compliant in line
with EWTD and New Deal.
 To ensure that adequate systems and procedures are in place to control and
monitor leave for junior medical staff and to ensure that there is appropriate
cover within the clinical areas, including on-call commitments.
 To participate in the recruitment of junior medical staff as and when required.
 To participate in team objective setting as part of the annual job planning
process.
 To be responsible for the annual appraisal of non-consultant grades as
delegated by the Clinical Director

Departmental and Specialty Information
The Mental Health Services in West Lothian are managed by the local Community
Health and Care Partnership. However they form part of the Lothian wide services
and are integral to the Lothian Mental Health and Wellbeing Strategy. All psychiatric
consultants working at St John’s Hospital are expected to engage in regional
planning for their particular specialty and relevant continuing professional
development activities.
In-patient Mental Illness services, including an Intensive Psychiatric Care Unit, and
Care of the Elderly with Mental Disorder beds are in St John’s Hospital, Livingston.
There are two regional inpatient services covering the South East of Scotland, a
Perinatal Psychiatry Mother and Baby unit and an Eating Disorders Ward.
Day patient services are provided at a Day Hospital for the Elderly with Mental
Disorder, at St John’s Hospital, and 5 Community-based Rehabilitation Day Hospitals
[including depot and Clozapine clinics]. A peripatetic service provides day care for
the Elderly with Mental Disorder at a range of community resources. Clinical
Psychology, Occupational Therapy and Psychiatric Nurses run a range of
psychotherapeutic groups in St John’s Hospital.
There is a wide range of out-patient clinics, both hospital and primary care based.
With Clinical Psychology Services and Community Psychiatric Nursing also based
within Health Centres, this offers the opportunity for close networking and liaison with
General Practitioners and other community staff. A community outreach team
comprised of Consultant Psychiatrists, Community Psychiatric Nurses and Social
Work staff provides a service to patients with a severe and enduring illness.
There is a dedicated team for Mental Health Social Work, and another for Mental
Health Officers and Adults with Incapacity.
The Clinical Psychology Department is also part of the CHCP. The department
offers dedicated sessions to Care of the Elderly with Mental Disorder, Child and
Family Psychiatry, in addition to 8 primary care based clinics.
Occupational Therapy and Physiotherapy Services are provided from within St
John’s Hospital by the respective departments. The Occupational Therapy service is
especially active in Mental Health.
Staffing
West Lothian Health and Care Partnership currently employ 12 Consultant
Psychiatrists, including 2.6 wte in the Care of the Elderly with Mental Disorder; 0.8
wte in Perinatal Psychiatry and 0.5 wte in Eating Disorders. Forthcoming
appointments will increase the consultant establishment with an additional post in
Care of the elderly with Mental Disorder.
Supporting Medical Staff
There are 2 Specialist Trainee posts based in West Lothian that are part of the South
East of Scotland Psychiatric Rotation. These posts offer training in Care of the
Elderly with Mental Disorder and Child and Adolescent Psychiatry. There is 1 core
trainee post. 3 General Practice Vocational Training posts and 3 Foundation Year 2
posts, providing experience in General Psychiatry and Care of the Elderly. There is a
Foundation Year 1 post in Liaison Psychiatry.
Specialty Trainee Registrar attachments are currently available in General
Psychiatry, Liaison Psychiatry, Rehabilitation, Care of the Elderly with Mental
Disorder and Child and Adolescent Psychiatry.
Organisation of Psychiatric Services
Adult Psychiatry including Rehabilitation
The model of general psychiatric care and the organisation of West Lothian services
was last reviewed in 2009. Clients with the most complex ongoing needs are cared
for by 1.4 wte Consultant in Rehabilitation Psychiatry, using services like the
Community Outreach Team and Day Services.
4.0 Consultants in General Adult Psychiatry are responsible for the acute care of
other patient groups and for a sessional commitment to locality based service
supporting primary care
Care of the Elderly with Mental Disorder
Two Consultants are currently identified as having a special responsibility for the
Care of the Elderly with Mental Disorder. They share responsibility for the 24 bedded
Assessment Unit within St John’s Hospital, the day-care services and a total 60
community based old age psychiatry beds for patients with dementia and challenging
behaviour. An additional post has recently been developed.
Liaison Psychiatry
A service is provided for St John’s Hospital at Howden by a Consultant Liaison
Psychiatrist. This includes the assessment of patients presenting after deliberate
self-harm.
Forensic Psychiatry
One Consultant is responsible for the running of the Intensive Psychiatric Care Unit
at St John’s Hospital. A supra-regional in-patient Forensic Unit is on the Royal
Edinburgh Hospital campus.
Child and Adolescent Psychiatry
The Child and Adolescent Psychiatry service based at St John’s Hospital is part of
the Lothian CAMHS service. A regional inpatient unit is part of the Royal Edinburgh
Hospital campus. A day patient unit operates in Livingston and an outpatient service
is offered at St John’s Hospital.
Substance Abuse Services
There is a regional service for people with Drug and Alcohol Problems which includes
a Consultant Psychiatrist. This provides dedicated community services in West
Lothian and in-patient alcohol services are provided at the Royal Edinburgh Hospital.
Perinatal Psychiatry
The East of Scotland Mother and Baby Unit is in St John’s Hospital. The out of hours
on-call rota for West Lothian will provide cover for this.
Eating Disorder Services
A new inpatient Eating Disorder Unit was opened at St John’s Hospital in 2012. The
out of hours on-call rota for West Lothian will provide psychiatric cover for this.
Learning Disability Services
These services are provided across Lothian, and West Lothian is served by a
community team with input from a Consultant Psychiatrist.
Population and Services
Catchment Area, Bed Numbers and Staffing
West Lothian has a total population of 180,000. The General Adult Psychiatry
Service covers all patients between the ages of 19 and 64 years, although there will
be continuing responsibility for some patients with functional illness over the age of
65. Patients under the age of 65 with possible dementia are generally initially
assessed by general psychiatry, who will usually initiate diagnostic investigations
before transferring to the old age service for follow up.
There is one in-patient general psychiatry ward in St John’s Hospital with 24 beds.
The responsibility for patients admitted to this ward falls on 4 general adult
psychiatrists. The consultants are responsible for patients admitted from their own
catchment area and they are allocated responsibility for patients admitted from out
with West Lothian on a rota basis.
There are separate in-patient wards for Intensive Psychiatric Care and Rehabilitation
that have their own consultants.
Acute Care and Support Team (ACAST)
This is a nursing team that are based in St John’s Hospital and who work with junior
medical staff in the urgent assessment of patients presenting to the Accident and
Emergency Department. ACAST also provide time-limited intensive home treatment
for patients with acute or relapsing psychiatric illness. General Adult Consultants
remain clinically responsible for patients from their catchment area, who are under
the care of ACAST
Outpatient Clinics
General Adult Psychiatry outpatient clinics are held at St John’s Hospital and at a
number of community bases including some GP surgeries. All non-urgent GP
referrals are discussed at primary care based triage meetings held fortnightly.
Research Audit and Teaching
Undergraduate medical students from Edinburgh University are attached to the Old
Age Psychiatry Department on a regular basis, for a period of 2 weeks.
The Mental Health Directorate has an active multi-disciplinary clinical audit group. All
medical staff within the hospital are expected to participate in Medical Audit, both by
their involvement in ongoing projects and by initiating review of their own work
practices.
The Consultant will be expected within his or her NHS responsibilities to undertake
research and to participate in undergraduate and postgraduate teaching as required.
As stated in the advertisement, the Consultant may be offered an appropriate
honorary appointment by the University of Edinburgh.
In certain circumstances, where the Consultant concerned has responsibilities for
organising teaching in a particular discipline or has a research interest which extends
beyond the duties of his/her NHS post, the Trust and the University may agree that a
Consultant be offered a part-time Senior Lecturer appointment with specific sessional
allocation. Such appointments will be subject to reconsideration by the Trust, the
University and the Consultant at intervals of not less than three years, and, in the
event of the contract being modified by the reduction or elimination of academic
sessions, the Consultant would be entitled to continue in the same number of total
sessions, all of which would then be devoted to Health Service work.
If the successful candidate wished to be considered for a part time Senior Lecturer
post, it would be necessary for the appointee to make special application to the
Division giving full details of the work to be undertaken.
There is an active psychiatric postgraduate programme at St John’s Hospital every
Tuesday from lunchtime during academic terms. Consultants and other members of
the teams present cases, journal clubs and there are also external speakers. There
are also regular CPD sessions.
5.
Job Plan and Conditions of Service
The job plan will be agreed with the successful candidate and approved by the
Associate Medical Director.
This is a 10 Programmed Activities post, with 9.0 sessions of Direct Clinical Care and
1.0 session of Supporting Professional Activities. The one SPA minimum will reflect
activity such as appraisal, personal audit and professional development occurring
outside study leave time. Once the candidate has been appointed more SPA time
may be agreed for activities such as undergraduate and postgraduate medical
training which takes place outside direct clinical care, as well as research and/or
management. These activities must be specifically and clearly identified and be
agreed with the candidate and desired by the department.
An appraisal system for reviewing personal development and job planning will be
undertaken annually.

Activities which are not undertaken at special locations or times
One week in 6 the post holder will work exclusively on the assessment and care
planning of patients admitted to the adult acute wards and providing urgent reviews
of patients referred from within the Mental Health Services, including Mental Health
Act (Care and Treatment) (Scotland) work.
The post holder will work with colleagues to support a community acute care and
support team outreaching from the adult admission wards.
The post holder will also have to complete work of an unpredictable nature in
relation to current patients detained under the Mental Health Act. This will include
completion of documentation and regular attendance at Mental Health Act
Tribunals.
Activities during Premium Rate Hours of Work eg hours out with 8am – 8pm Monday
to Friday
The post holder will provide out of hours consultant cover on a 1:10 rota.
He/she will be available to carry out urgent assessment of patients at St John’s
Hospital and Livingston Police Station if required.
At weekends, the on-call consultant will assess all patients admitted under
emergency detention orders at the earliest opportunity.
A formal Job Plan will be agreed between the appointee and their Clinical Director,
on behalf of the respective medical management structures within NHS Lothian. The
job plan will be based on the provisional timetable shown. The Job Plan will be
reviewed annually, following the appraisal meeting. The Job Plan will be a
prospective agreement that sets out a consultant’s duties, responsibilities and
objectives for the coming year. It should cover all aspects of a consultant’s
professional practice including clinic work, teaching, research, education and
managerial responsibilities. It should include personal objectives, including details of
their link to the wider service objectives and details of the support required by the
consultant to full the job plan and objectives.

MEDICAL
NEGLIGENCE
In terms of NHS Circular 1989 (PCS) 32 dealing with Medical
Negligence the Health Board does not require you to subscribe to a
Medical Defence Organisation. Health Board indemnity will cover
only Health Board responsibilities. It may, however, be in your interest
to subscribe to a defence organisation in order to ensure you are
covered for any work, which does not fall within the scope of the
indemnity scheme.

SOCIAL
POLICY
Employment is subject to three months’ notice on either side, subject
to appeal against dismissal.
BASE You may be required to work at any of NHS Lothian’s sites as part of
your role.
MEDIDA You are required to adhere to NHS Lothian’s Social Media policy
which highlights the importance of confidentiality, professionalism and
acceptable behaviours when using social media. It sets out the
organisation’s expectations to safeguard staff in their use of social
media.
