2. JOB PURPOSE
As part of a multidisciplinary team, the post holder will have responsibility for assessment of
health and psychological needs of Older Adults admitted as an emergency to the Acute
Assessment Units, and in specialities outside Medicine of Elderly Directorate.
The main aim of the multidisciplinary team is to ensure all older frail adults access early and
appropriate specialist assessment to facilitate the delivery of high quality care wherever the
patient is placed within the acute hospital
The post holder will provide specialist advice to healthcare professionals, other agencies,
clients, carers and relatives
The multidisciplinary team will expedite the move of patients to an specialist MOE setting where
required, and will , also support the specialty teams in the coordination of the patient pathway
facilitating a safe and timely discharge directly back to the community when appropriate.
The Nurse Practitioner will work autonomously as an expert practitioner with support from a
Medicine of the Elderly Consultant.
3. DIMENSIONS
The Post Holder will work within the Medicine of the Elderly team. Using higher levels of
decision making and clinical judgement deliver early screening of older patient at the ‘front door’
to ensure patients are admitted to areas most appropriate for their need, or have appropriate
follow up plans if discharged directly to community.
The post holder will review patients within specialty care as requested to ensure aspects of
comprehensive geriatric assessment are instigated and to provide advice and education to other
speciality clinical teams, and assess suitability of older patients within specialty areas for a move
to MoE setting.
The post holder will provide specialist advice to healthcare professionals, other agencies,
clients, carers and relatives, and may have junior staff/learners working with them.
The post holder has professional responsibility and is accountable for their actions at all times
and is expected to work within the parameters of their knowledge.
The post holder is not responsible for managing the budget, but needs to be aware of the
resources available and the need to work within the financial envelope.
The post holder will develop strategies to improve practice and work collaboratively across
healthcare boundaries, implementing and evaluating evidence based standards, guidelines and
policies.
The post holder may be required to work within any of the Lothian Acute Services pertaining to

5. ROLE OF DEPARTMENT
The Medicine of Elderly Directorates aim is to provide a high quality, safe and supportive
environment in order to care for patients, meeting the identified physical and psycho-social
needs.
There are 154 dedicated Medicine of Elderly beds over 6 wards within Medicine of Elderly
Directorate on Western General Hospital site and an Assessment and Rehabilitation Centre.
There is also a 40-bedded stroke unit.
WGH has a high number of acute medical and surgical speciality beds with significant numbers
of frail older patents cared for in these areas.
There is currently a small dedicated Multidisciplinary Medicine of Elderly Liaison Team (known
as Team 65/ COMPASS) where this post will reside. The major role of the team is to provide
specialty assessment for older patients who are cared for out with the MOE bed compliment.
Key aims of the team are:

To develop proactive screening of older patients being admitted to front door services, and to
specialty areas, with the aim of instigating early specialist and individualised assessment and
management of older adults according to clinical need.

To ensure all appropriate older adults access early Comprehensive Geriatric Assessment
through a co-ordinated multidisciplinary team approach.

To work as part of the team facilitating patient admissions and discharge through enhanced
community linkages developed by COMPASS working pattern.

To deliver direct clinical care to older patients within a ‘virtual ward’ as part of a MOE
consultant led ‘mobile’ multidisciplinary team.

To ensure the older patients pathway is well co-ordinated with clear communication with
patient, relatives and between secondary and primary care clinical teams.

To ensure safe, high quality care delivered with compassion and tailored to the individual
patient’s needs and wishes.

To facilitate safe and timely discharge form secondary care with no unnecessary delays.

Act timeously in resolving local complaints effectively through investigating and responding
