If Accepted to participate in the Storm FC 3v3 Soccer Shoot Out Tournament I/we hereby waive, release, indemnify and agree to hold
harmless the Storm FC, Cheney School District, City of Cheney, officials, referees, sponsors and volunteers for any claim arising from any
injury to my child.
