Maternal and Child Health Mission Statement: The Mission of the Nevada County Public Health department is to work both as a single entity and in coalition with other health and human service providers to promote health, prevent disease, and improve and prolong the quality of life for all Nevada County residents. Furthermore, the mission of Maternal and Child Health is to promote the health of Nevada County’s infants, children, youth, adolescents, and mothers. Service Description: Provide Maternal, Child and Adolescent Health (MCAH), Women Infants and Children (WIC), and Children’s Medical Services (CMS) program services. Services provided focus on: (1) promoting the health and well-being of all women of childbearing age, infants, children, and adolescents and providing comprehensive outreach activities to the MCAH population; (2) identifying and qualifying Nevada County children with physically disabling medical conditions for the California Children’s Services (CCS) program; (3) providing administrative services for the State Child Health and Disability Program (CHDP) program at the county level so that low-income children in Nevada County have access to periodic preventative medical care; (4) ensure that children in Foster Care receive appropriate medical care and that medical information will follow the child even if (s)he is transferred from county to county; (5) protect the health of women, infants, and children by providing nutrition education, promoting and supporting breastfeeding, helping to decrease the complications of pregnancy, preventing low birth weight babies, decreasing iron deficiency anemia, and promoting optimum growth and development of infants and young children; (6) educate the community about the dangers of drug use/abuse particularly during pregnancy. Major Accomplishments in 2008-2009: MCAH PHNs provided home visitation services to an average 45 mothers and their infants, per month. NEVADA COUNTY BUDGET 2009-10 Over 700 mother-infant home visits were provided by MCAH PHN’s. Western Nevada County 2008 annual MCAH referrals of 143 increased 42% over 101 referrals received in 2007. Maintained and grew new collaborative relationships with public and private agency partners and participated in over 25 interdepartmental and agency meetings by 12/08. Standardized client registration and MCAH PHN medical record documentation between eastern and western Nevada County. Hired and oriented two new MCAH PHNs to fill vacated positions. WIC implemented a baby sling project with funding from First 5 Nevada County, collaborating with other agencies to give 94 infant slings to women to promote breastfeeding, improve infant development, and prevent “flat head” syndrome. WIC Coordinator, WIC Lactation consultant, MCH Coordinator, and Public Health nurses were active participants in the Nevada County Breastfeeding Coalition. Breastfeedingfriendly crib cards and breastfeeding information packets for use at Sierra Nevada Memorial Hospital were provided in both English and Spanish. They also set up and staffed a Baby Feeding and Changing station at the Kids Korner booth at the Nevada County Fair. Maintained WIC participation levels at 100105% of caseload, or 1,575 participants. WIC caseload increased from 1,475 to 1,575 on 10/01/08. Maintained rates of exclusive breastfeeding for WIC infants (birth to age 1) at 32%. CCS processed 320 referrals in FY 08/09. CCS provided an average monthly 360 children with specialty medical and therapy services. CCS provided 80 children with therapy services. CHDP provided an estimated 3,500 CHDP Health Assessments and follow-up for low income children. 2-314 Maternal and Child Health Obtained medical passport information for 35 newly detained foster children. Maintained current CHDP exams on 75% of caseload and dental exams on 60% of caseload. Objectives & Performance Measures for 2009-10: Objective: Provide professional and skilled public health nursing (PHN) expertise to pregnant and postpartum women and their babies using established guidelines and standards that have been proven effective in public health practice. Performance Measures: Policies and procedures are in use to provide expert nursing care to new mothers and their infants by 12/09. Revise, develop and implement cohesive and interchangeable nursing charting and care plans between Truckee and western Nevada County by 12/09. All MCAH Public Health Nurses will have received Denver Developmental Screening Test II Training within one year of date of hire. Objective: Coordinate and enhance collaboration with public partners and private agencies that provide services to mothers and their families, with an outcome of providing a seamless continuum of care. Performance Measures: By 12/2009, participate in 25 interdepartmental and agency meetings. Maintain the current number of MCAH PHN referrals within Nevada County at 140. Objective: Work collaboratively with community partners toward reducing the incidence of drug use, especially methamphetamine usage by parents, through education and cooperative efforts with other county and community groups. Performance Measures: Provide one-day-per-week classroom instruction/participation in the Parenting Class at Park Avenue High School for at least 25 hours per school year. NEVADA COUNTY BUDGET 2009-10 Participate in two public presentations, partnering with the Coalition for a Drug Free Nevada County by 12/2009. Objective: Encourage breastfeeding, healthy nutrition and contact with needed community services in families with young children according to State Women Infants and Children (WIC) program guidelines by providing education and food vouchers. Performance Measures: Provide monthly WIC food vouchers to 1,575 WIC participants as directed by the State WIC Program. Monitor and maintain rates of exclusive breastfeeding for WIC infants (from birth to age 1) at as high a rate as possible, aiming for 31-34% of mothers exclusively breastfeeding infants under 1 year. Provide at least 300 nutrition classes during FY09-10. Objective: Continue to provide State-mandated Child Health and Disability Program (CHDP) Healthcare program for Children in Foster Care (HCPCFC) and California Children Services (CCS) services to children ensuring that low-income children and foster children in Nevada County have access to periodic preventative medical care and physically disabled children get the medical services they need. Performance Measures: Process 320 referrals to CCS. Provide an average monthly 350 children with medical and therapy services through CCS. Provide 80 children with CCS Therapy services. Provide 3,500 CHDP Health Assessments and follow-up for low-income children. Conduct 4 outreach activities to general public through local media, schools, other Nevada County programs, and other appropriate avenues regarding CCS and CHDP availability. Provide 100% medical follow-up of new foster detainees, 75% annual CHDP exam compliance and 75% dental compliance. Provide 2 trainings for CHDP providers. 2-315 Maternal and Child Health Participate in 5 kindergarten registration roundups.
