Policy: Each Local Agency (LA) is required to perform a Self Evaluation each fiscal year to review clinic operations. This evaluation shall include a review of the following areas: Clinic Environment, Certification, Nutrition Education, Breastfeeding,

Participant-Centered Services, Food Delivery, Food Instrument Security, and Civil

Rights. The purpose of the review is to ensure that WIC services are provided in accordance with State of Nevada WIC policies and Federal regulations.

Procedure:

1. Evaluations are to be conducted annually at all LAs.

A minimum of 50% of clinics shall be reviewed within each LA. 2.

3. LAs shall utilize the Local Agency Self Evaluation Checklist and the PCS

Observation Form, as well as follow the Quality Assurance Standards in policy PE:3 as a guideline for review. Certification, Nutrition Education,

Breastfeeding and Food Delivery Standard Procedures may be completed as part of the LA ’s staff work performance evaluation.

4. The Self Evaluation shall consist of a combination of participant file reviews, observations of CPA interactions with participants and review of those charts, and observation of clinic environment and procedures. A minimum of 3 participant file reviews per WIC category (Prenatal,

Breastfeeding, Post Partum, Infant, and Child) and 1 observation per category for each Agency CPA shall be conducted and kept in a clinic file for State Program Evaluation purposes.

5. Procedure items for inclusion: Clinic Environment; Certification, including

Eligibility, Anthropometrics, Hematology, Health/Nutrition Assessment,

Nutrition Education, Breastfeeding, Participant-Centered Services,

Referrals; Food Delivery, and Civil Rights.

6. Documentation of the LA Self Evaluation shall be kept on file at the LA for a minimum of three years following the date of the self audit. Documentation must also include the date the records were reviewed, the ID numbers of the participants and the names of the reviewers. All documentation shall be made available to State personnel upon request.

7. In areas where deficiencies are found regarding Clinic Environment,

Certification, Food Delivery, Food Instrument Security and Civil Rights corrective action shall be made. In areas where deficiencies are found regarding Nutrition Education, Breastfeeding, and Participant-Centered

Services, suggestions for improvement, staff mentoring and further observations shall be made toward staff development.
