Please confirm you are in agreement in the case of emergency and we are unable to
contact you that if medical treatment is required we can use the services of a vet at your
expense. ___________In all cases we will contact you first with any worries regarding
health that we may have. In all cases we will only incur vet expenses that are necessary.
Please confirm that you fully understand and accept that should your pet become ill and
any required treatment is unsuccessful and your pet dies whilst in our care that Select Pet
Centre will not be held responsible for your pet’s death and that Select Pet Centre will not
accept any claim for the loss of your pet.
I agree that if I am not able due to unforeseen circumstances to collect my pet(s) on the
collection date I will contact Select Pet Centre immediately to advise. I understand that
should I fail to collect my pet(s) at the required time that I will be responsible for any
additional boarding charges. Select Pet Centre reserves the right if your pet(s) is not
collected and no contact has been made with us that we may re-home your pet after a
reasonable period of time. We would of course make all attempts to contact yourselves
with your contact details provided.
Select Pet Centre agrees to take good care of your pet(s) at all times and ensure that all
requirements are met satisfactorily. In the unlikely event that your pet escapes or is
stolen we are unable to insure against this consequence. We therefore ask that you take
out your own insurance to cover for escape or theft of your pet(s). Please understand we
will not be held liable for escape or theft or your pet(s).
Please advise us of anything we should be aware of in connection with your pet(s) no
matter how small so that we may ensure we are able to look after him/her correctly

All rabbits must have current vaccination certificate for myxomatosis and VHD. A
photocopy or Certificate needs to be produced at time of booking your rabbit(s) in.All
guinea pigs/rabbits must have been treated recently for mites. We are able to do this with
‘spot on’ if the animal has not been treated and a small charge will be made for the ‘spot
on’. All animals must be healthy when coming in for boarding. We are happy to
administer long term medication from the vet. We reserve the right not to take an animal
in for boarding if we feel it is unwell.
