nos. of private Ambulance with drivers helpers stretcher and oxygen cylinder for
round the clock for 24 hrs. a day from 6 AM to 6 AM at Andal Railway Hospital for
three years by outside agency

TENDER FORM FOR HIRING OF AMBULANCE

On behalf of the President of India, Chief Medical Superintendent, Eastern Railway, Andal invites
sealed Tender for Hiring of 02 (two) no’s of private Ambulances with Driver’s, Helper’s
Stretcher and Oxygen Cylinder for round the clock for 24 hrs. a day from 06 AM to 06
AM at Andal Railway Hospital for three years by outside agency.

Tender forms containing full details can be had in persons from the office of the Chief Medical
Superintendent, Eastern Railway, Andal on production of receipt of Rs.2000/- (Rupees two
thousand only) (Non-refundable) which is to be deposited with Chief Cashier/ Eastern Railway/
Kolkata or Sr. Divisional Cashier/Eastern Railway/ Asansol. Money Receipt should be deposited
in original for obtaining tender form. Tender Form is not transferable.
Tender forms will be sold from 01.03.14 to 20.03.2014 during office hours. Tender will be received
on or before 13.30hrs on 21.03.14. The same will be opened at 16.00hrs. on 21.03.14.
Tender addressed to Chief Medical Superintendent, Eastern Railway, Andal in sealed cover
should be dropped in the Tender Box to be kept available in front of the office of Chief Medical
Superintendent /Eastern Railway/ Andal on or before 13.30hrs. on 21.03.2014 duly filled in having
signature in each pages.
Tender unaccompanied with full Earnest Money will under no circumstances be entertained and
will summarily rejected.
Earnest money of un-successful tenderer will be returned on demand without interest & earnest
money of successful tenderer may be adjusted towards security deposit
Introduction of performance Guarantee:a)
The procedure for obtaining performance Guarantee is outlined below:The successful bidder should give a performance Guarantee in the form of an irrevocable bank
guarantee amounting to 5% of the contract value.
b) The performance Guarantee should be furnished by the successful- contractor after the letter
of acceptance has been issued ,but before signing of the agreement and should be valid up to
expiry of the maintenance period. The agreement should normally be signed within 15(fifteen)
days after the issue of LOA and performance Guarantee should also be submitted within this time
limit.
c) Performance Guarantee shall be released after satisfactory completion of the work and
maintenance period is over. The procedure for releasing should be same as for security deposit.
d)
Wherever the contracts are rescinded, the security deposit should be forfeited and the
performance Guarantee shall be encased and the balance work should be got done separately.
e)
The balance work shall be got done independently without risk and cost of the original
contractor.
The original contractor shall be debarred from participating in the tender for executing the
balance work. If the failed contractor is a JV or partnership firm, then every member/ partner of
such a firm would be debarred from participating in the tender for the balance work either in
his/her individual capacity or as a partner of any other JV/partnership firm
.

7 A On receipt of the letter of acceptance of tender from the Railway Administration the contractor
shall within a period of 7 (seven) days from the date of receipt of the said letter of acceptance
deposit security money per one (01) lakh in value 10 % of the value of contract for costing more
than I lakhs-10% of the 1st one lakh & 7.5 % of the balance. For contract costing more than 2
lakhs, 10 % of the 1st one lakh 7.5% for next one lakh & 5 % of the balance subject to maximum
of Rs1.5 lakhs. The earnest money may be adjusted against security money.
Security money may be deposited in any of the following forms:a) In cash to be deposited with the Chief Cashier/ER/Kolkata or
Sr. Divisional Cashier /ER/Asansol
b) Deposit receipt
c) Pay Order
d) Demand draft
e) Fixed Deposit receipt
f) A deposit in Post Office Savings Bank
g) 12 years National Defence Certificate
h) 10 years Defence Deposit.
i) National Defence Bond.
All the Certificates should be pledged in favour of FA & CAO/ E.Rly/ Kolkata

Security deposit will remain with the Administration free of any interest & will be released after
the contract period is over.
You are requested to submit a certificate regarding working & financial credential and also that
no relatives of yours are employed in the Railway or any retired Gazetted Officer is employed in
your office. Also latest ITCC & STCC should be submitted along with the offer.
The tenderer hold the offer opens for a period of 120 days from the date of opening the tender. It
is understood that the tender document have been issued to the tenderer and the tenderer is being
permitted to tender in consideration of the stipulation on the part after submitting his tender, he
will not reside away from his offer or modify the terms and conditions there of in a manner not
acceptable by Divisional Railway Manager(IRS) Eastern Railway, Asansol, if the tenderer fail to
observe or comply with the foregoing stipulation the amount deposit as security for the due
performance of the above stipulation shall be forfeited by the Railway. If the tenderer is accepted
the amount of earnest money will be held as security deposit for the due and faithful fulfillment of
the contract. This shall be forfeited in the event of failure to execute the contract as per stipulation
of premature termination of the contract.
A full value of earnest money shall stand forfeited without prejudice to any other right or
remedies if
a) Contractor does not execute agreement within 15 days from the date of receipt of
notice issued by the Railways.
b)Contractor does not supply the vehicle within 15 days after receipt of order tothat effect.
Penalty for the non availability of van will be at three times of the approved rate.
Variable rate-running expenses/KM of usage of vehicle on any type of road i.e. Kancha/ Pacck in
cities/in villages/in jungles etc. The rate is inclusive of all taxes license, fees, permits, fuel, POL etc.
complete in all respect.

Payment will be made through account payee cheque to be issued by Sr. DFM/ASN on submission of
monthly bill through CMS/UDL.
The contractor shall be responsible for the general conduct & discipline of his labourers . He shall be
penalized by way of fine or otherwise by dismissal any of his workman if so required by the
CMS/ER/UDL or his authorized representative on account of unsatisfactory work in subordination or
any other cause.
i) Driver should have valid licence. Vehicle must have updated blue book. The contractor should
provide the vehicle which is not more than (03) three years old. The contractor is required to
maintain the vehicles in a satisfactory state at his own cost. The vehicle should have a decent look and
sheet have proper cushioning with back rest. The vehicle would be subject to monthly inspection by
suitable Railway authorities. The deficiencies pointed out by the Railway representative will have to be
rectified by the contractor. The vehicle and driver should be offered round the clock every day after
proper cleaning. The Railway authority has the right to impose penalty/interim legal action for
violating the condition.
ii) From safety of view the drivers of the vehicle should work in 8.00hrs. shift.
iii) They should not be intoxicated. Any checking (e.g. Breath Analyzer testing) from the side of Railway
administration may be done at any time and it a driver is found positive in B.A. testing, a police case
may be lodged
The contractor would be required to provide reliable transport round the clock on every day of week
with no holidays. If vehicle is required to be withdrawn for repair, maintenance, brake down, it would
be immediately replaced by suitable vehicle. No trip is to be cancelled on account of non-availability of
vehicle and the contractor is expected to provide replacement vehicle within half an hour.
In the case of contractor’s failure to make the trips, the railway reserve the right to hire vehicle at the
prevailing market rate and put it into service and cover the expenditure from the contractor’s
bill/security deposit.
In case of repeated failure of unsatisfactory performance railway reserves the right to terminate the
contractor in accordance with laid down conditions and forfeit the entire security deposit of the
contractor.
The transport driver will maintain a log book of all the trips made by the vehicle and would have to get
the signature at the end of the trip.
The contractor will not allow any person not authorized by the railway to travel by the vehicle.

The contract will remain in force for a period of one year from the date of commencement. However
the Railway Administration may terminate the agreement by giving to the contractor 45 days notices
without assigning any reason thereof & in the event of any breach of any condition here in contained
by the contractor or in the event of misconduct on the part of the contractor of his workman. It shall
be lawful for the CMS/ASN to terminate this agreement for without any notice and to forfeit the
whole or part of security deposit at his discretion without being liable to the contractor for any loss or
damage what so ever.

The contractor should submit along with the income tax clearance certificate in original or authorize
copy thereof if he has not taxable income, a sworn of affidavit duly countersigned by the Income Tax
Officer to that effect. The contractor shall submit testimonials along with their sealed tender in regard
to their past experience in hiring job to Central Govt./State Govt. or other organization.

This will be a running contract for a period of one year from the date of commencement of the work
and can be extended by one year at the discretion of the Railway Administration on the same rates and
condition of the work.
Income Tax will be deducted from the bill as per extant rules.
The vehicle provided by the contractor must be fitted with Type-A oxygen cylinder
and refilled by the contractor whenever it is exhausted.
The validity of offer should be kept open for 120 days from the date of opening of tender.
On requirement in case of any abnormality/accident the vehicle may be used at other place/locality for
the interest of the Railway Administration without charging any extra cost.
The contractor must have a Mobile Phone Number which is to be given to Railway authority at time of
agreement for immediate contact by the Railway Authority as & when required in case, if any
defect/change in Mobile connection, the contractor should immediately inform to the Railway
Authority.
In the event of any differences or dispute what so ever at any time arising under the terms &
conditions of contract or any other manner under the contract or any relating there to or as to the true
meaning or interpretation of the provision thereof (except as to any matter the decision of which is
specially provided by these conditions or any other provision in the contract or for which the
contractor has submitted no claim certificate). The same shall be referred to the General Manager
who shall thereafter nominate the arbitrator in such differences/dispute. Who ever is so nominated
shall be the sole arbitrator & the decision of the arbitrator will be final, binding and conclusive and
the provision of the arbitration & conciliation act 1996 (Act 26 of 1996) of the rules there under and
any statutory modification thereof shall be deemed to apply and be incorporated in the contract.

Other terms and condition will be as per general condition of contract.
The financial acceptance of the tender shall rest with the CMS/AR/ASN who deserves the
right to accept any tender of reject any tender or all tenders without assigning any reason &
does not bind himself to accept the lowest tender only.
