Please carefully review the different scholarships on page 2 and check all that you qualify
and applying for. You may choose more than one, but can only receive one scholarship.
This application must be filed with the NAM office. The final date for filing completed applications is May 25, 2015.
NAM, parish and MYO involvement, scholarship, and leadership are the criteria by which applicants will be judged. Financial need is not a factor in the selection of scholarship winners.
All scholarships are in the form of certificates of award issued by the NAM office, conditioned upon the enrollment of the student in an undergraduate four-year degree program or a graduate program in an accredited college or university. Upon receipt of "Verification of Enrollment", from proper school officials, a NAM Scholarship check for the amount of the award will be forwarded to the college or university to establish a credit for the student.
All applications become the property of the National Apostolate of Maronites.

GENERAL INSTRUCTIONS
1.
A copy of your High School transcript of grades from the beginning of your High School matriculation or a copy of your college transcript of grades from the beginning of your college matriculation to date must be included with the application.
2.
A letter of reference from a high school teacher or advisor, or a letter of reference from a college professor or advisor along with your pastor's recommendation form must accompany this application.
3.
A copy of your College Entrance Exam Board scores (ACT/SAT) if not included on your transcript (for the high school seniors scholarships)
4.
A copy of a face picture (digital preferably) to be used during the scholarship ceremony on
Thursday night at the National Convention.
5.
Your completed application along with the above required supporting documents must be given to your pastor in order to reach the NAM Office no later than May 25, 2015.

Eligibility Requirements
Applicant must
1.
Be a current high school, or college student; depending on the scholarship you are applying for
2.
Be a member of a Maronite Parish, MYO and preferably a NAM member
3.
Not be a recipient of a previous NAM Scholarship.
4.
Have maintained a college 3.0 GPA (based on a 4.0 scale)

8) List of achievement in scholarship, leadership, athletics, MYO activities, MYO activities, community service, or other activities may be attached - but avoid being repetitious

The announcement of winners will be made on Thursday night at the Scholarship Ceremony at the annual NAM Convention.
