2. ASSESMENT DESIGN EVALUATION
Moderator Report confirming that the moderator has evaluated the design of assessment tools and the planned
assessment activities. The assessment strategy, plan and methods used are clearly defined and that they meet
principles of assessment listed below:

3. ASSESMENT PROCESS EVALUATION – V2
Required from the Moderator
a. The moderator is expected observe the assessor conducting assessments to ensure and confirm that
assessment plans were followed and the above aspects addressed. Only a sample of the assessment
activities should be observed.
b. The following should be considered:
-
A pre-assessment discussion was conducted. And the candidate acknowledges their rights, roles and
responsibilities.
-
The assessment plan (Including time frames) is agreed by the candidate and the assessor prior to
assessment.
-
The assessment methodologies and activities are agreed by the candidate and the assessor prior to the

Documented assessment methods are compiled and readily available for all outcomes that need to be
assessed.
-
The assessor informed the candidate of the evidence requirements in relation to the unit standard
-
The assessor explained what the candidate and the assessor would be doing during the assessment
-
The candidate acknowledge the authenticity of the evidence they submitted
-
The assessment was conducted during circumstances that reflect realistic or lifelike conditions
-
There is an indication that reasonable arrangements had been made for candidates with disabilities.
-
Assessments were conducted without any obtrusive disruptions to the candidate.
-
The assessor accurately evaluated the candidate’s evidence during the assessment, where appropriate.

Candidate had access to an appeal procedure if they wished to challenge assessment decisions.

The assessor gave clear and constructive feedback during and following the assessment decision

4. EVIDENCE EVALUATION FORM – V3
The moderator is required to evaluate the learner evidence and see if they meet the qualities listed below.

Did the learner have any difficulties in performing the assessment due to circumstantial reasons such
as: a lack of resources, or time, fatigue after night shift
b. The evidence was AUTHENTIC

Is there proof that the evidence was the candidates own work?

Were testimonials of observed work signed by a responsible individual
The evidence was CURRENT

Did the evidence relate to the latest version of the unit standard
Does the evidence relate to current competencies

Does the evidence reflect consistent competence across different circumstances
e. The evidence was SUFFICIENT

Is there evidence for ALL selected specific outcomes and assessment criteria?
Is there evidence for the ENTIRE selected range statements
The outcome is reached by moderating a sample of assessment results and POE of learners that have completed
assessments.
The moderator must indicate if he has accepted the assessor report and endorsed the decision reached by the
assessor. Indicate if any problems and weaknesses were identified and recommendations made to rectify them
and whether the corrections were conducted and accepted by the moderator.
Submit a copy of assessor report on all the learners confirming achievements and it must be co-signed by the
moderator.
Copies of reports and checklists used during the evaluation should be added as evidence.
5.
QUALITY ASSURANCE OF LEARNER ACHIEVEMENT CHECKLIST – V5
The moderator should evaluate the record keeping system to find out if the system is adequate and if there is
policies established for record keeping. Indicate if any problems and weaknesses were identified and
recommendations made to rectify them and whether the corrections were implemented.

All assessed learners are enrolled with the provider or workplace provider

A database for the recording of learner information, achievements and certification is in place

6. MODERATION PROCESS
Required from the Moderator
- Submit a moderation strategy and plan followed during the moderation process
The moderator plan and strategy should address the following.
 Moderation plan which complies with the ETQA requirements
 Moderator has provided feedback to the assessor on the moderation findings
 Evidence that the assessment tools were moderated prior to the assessment
 There is a system in place to audit, moderate and update the validity, currency and accuracy of the assessment
tools on an ongoing basis
 Moderation results and findings of the moderation are recorded
 The moderation process includes consideration of
 The assessment tools
 The assessment process
 The assessor
 The assessment results
 The moderator’s results include recommendations for non-conformance areas.
 A valid and representative sample of assessments was moderated according to ETQA sampling criteria.
 An appeals & grievance policy and procedure for assessments are in place
