Emerging Infectious Diseases (EID) Fellowship
Host Laboratory Application Instructions –
State/Local Laboratories
Overview and Instructions: State, local, or federal (CDC) public health laboratories
may apply to host EID training and/or research fellows. The host laboratory and staff
must demonstrate the ability to provide training and/or research opportunities for the
appropriate duration as described in the fellowship program timeline. There is no
guarantee that an eligible laboratory will receive a fellow. Please evaluate the following
responsibilities and requirements. For consideration, complete and sign the application
forms. Sections of previous host laboratory applications cannot be substituted for the
2011 application. A new application must be submitted for consideration.
Applications must be submitted electronically (email to fellowships@aphl.org) by
March 1, 2011. Signature pages must be faxed or mailed to APHL and must be
received by March 1, 2011. See the attached program timeline for more details.

HOST LABORATORY RESPONSIBILITIES AND REQUIREMENTS
The host laboratory must:
1. Have broad laboratory capabilities to ensure fellows are familiar with all relevant
techniques in a given specialty.
2. Demonstrate adequate laboratory research capabilities in infectious diseases.
3. Have demonstrated ability to provide training and/or research opportunities.
4. Provide opportunities for the fellow to participate in a field investigation or
special laboratory assignment to assist in the investigation of a disease outbreak.
5. Ensure the fellow participates in “research in progress” or other relevant seminar
series or teleconference.
6. Provide an on-site mentor who will inform, instruct, and provide an example for
the fellow. The mentor will assist the fellow as a functional member of the
laboratory and provide a stimulating atmosphere in which to learn and conduct
research.
7. Designate an on-site “facilitator” to interact with the fellow, mentor, and host
laboratory personnel. This individual will assist the fellow in situating to the area,
including locating local housing and transportation.
8. Supply the fellow with appropriate office and laboratory space, facilities,
technical and clerical support (including an email account), and access to
necessary equipment and personnel.
9. Be a member of APHL.
MENTOR RESPONSIBILITIES AND REQUIREMENTS
The mentor will be a highly experienced public health laboratorian who will guide and
direct the fellow for the duration of the program. The mentor will work at the host
laboratory and be responsible for the academic, technical, and ethical development of the
fellow. The mentor must meet the following requirements:

Hold a supervisory level laboratory position.
Have research or training experience/goals complimentary to the fellow’s project.
Be available and accessible for the duration of the program.
Provide a realistic appraisal of the fellow’s goals and performance based on
objectives agreed upon at the beginning of the assignment.
5. Provide constructive criticism and enthusiastic support to the fellow.
6. Provide periodic written reports on the fellow’s progress.
APPLICATION MATERIALS
The application consists of a cover page and three (3) sections. Section A is for general
information about your laboratory. Section B is the proposed project(s) [formerly called
“billet(s)”]. Section C is for information about the mentor(s). If you propose multiple
projects and/or mentors, copy and paste additional Sections B and C in your application.
Section A: Host Laboratory Information
Complete all sections. The laboratory description must total no more than two (2) singlespaced typed pages.
Section B: Proposed Project(s) [formerly called “billet(s)”]
Describe the fellow’s proposed training or research experience. Include projects and/or
training activities in which the fellow will be involved, and expected outcomes
(publications, outbreak investigations, etc.) Projects will be reviewed by candidates and
are a major factor in their host laboratory selection. At the top of each project, specify
whether you would consider only a training fellow, only a research fellow, or either type
of fellow. To maximize your chances of attracting a fellow, we encourage you to
consider both levels of fellows. If you specify one type of fellow on your project, APHL
will request only those fellows consider an opportunity in your laboratory.
Each mentor may submit up to two (2) projects for consideration (multiple copies of
Section B may be submitted). Each project must be no longer than one (1) single-spaced
typed page. No more than two (2) fellows may be assigned to the same mentor.
Section C: Mentor Application
Host laboratories may propose more than one mentor (multiple copies of Section C may
be submitted). Mentors are required to submit an application form, a description of their
experience and responsibilities (one page maximum), and a CV (two pages maximum).
Host laboratories are required to identify an alternate mentor to continue the program if
the primary mentor is unable to complete his/her responsibilities. Although both the
primary and the alternate mentor are required to sign Section C, only the primary mentor
is required to submit supporting material.
APPLICATION PROCESS:
Host laboratory applications must be submitted electronically (as a word document) and in
their entirety by March 1, 2010, to APHL at fellowships@aphl.org. Signature pages must
be submitted by fax or mail and received by March 1, 2011. Send signature pages to:
