Job Description – HIV Clinical Pharmacist DEPARTMENT: REPORTS TO: Client Services Client Services Manager SALARY AND BENEFITS Current FTE: 10 hours/week Salary starting wage: $52 THE POSITION In accordance with all HIV Alliance policies and procedures, the Pharmacist provides treatment adherence counseling to clients enrolled in case management services. The goal of treatment adherence counseling is to ensure readiness for, and adherence to, complex HIV/AIDS treatments. This enhanced treatment adherence counseling service will be provided by a licensed pharmacist through the State of Oregon, and includes the following key activities: (1) initial in-depth evaluation and assessment of barriers to and reasons for non-adherence; (2) development of a comprehensive, individualized Treatment Adherence Plan; (3) treatment adherence counseling interventions; (4) ongoing client monitoring to assess the efficacy of the Treatment Adherence Plan; and (5) periodic reevaluation and adaptation of the Treatment Adherence Plan as necessary. This includes all types of counseling interventions (both individual and group) including face-to-face, phone contact, and any other forms of communication. The pharmacist will assist the clients in defining and achieving agreed upon goals, and will track and monitor progress using a variety of tools. The Pharmacy team at HIV Alliance includes two clinical pharmacists and a Pharmacy Assistant who assist the Pharmacists with administrative duties. . RESPONSIBILITIES Complete treatment adherence assessment on eligible clients Collaborate with clients to develop treatment adherence plans Recommend interventions for improvement of medication adherence Provide client education pertaining to HIV and other medications, disease progression, and risk reduction] Perform drug interaction checks on client medication lists Review medication list for appropriateness of therapy Recommend pertinent adjustments to providers for simplification of medication regimen, maximization of therapy, and safety Build and maintain relationships with medical providers D:\116100997.doc Conduct case conferences with Ryan White Care coordinators, medical providers, clinical pharmacists, and HIV specialists Advocate for clients with medical service providers and case managers Provide direct one-on-one pharmacy adherence services to eligible clients Maintain up-to-date knowledge of disease progression and relevant therapies Maintain up-to-date and accurate client records Track and monitor clinical outcomes Provide intensive, short-term intervention to newly diagnosed clients focusing on HIV disease management, prevention/risk reduction education Participate in necessary meetings Assist Director/Manager with all necessary reports Participate in agency teams as requested Other duties as defined by Director and Ryan White Principles and Standards QUALIFICATIONS Valid Oregon Pharmacy license Valid Oregon driver’s license, driving record sufficient to be covered by agency auto insurance policy, ability to transport self to job-related events and education locations TB test Flexible hours required Ability to pass agency criminal background check Ability to work effectively and communicate with diverse groups of people Experience in clinical counseling regarding medication therapy and management Certification and/or experience in HIV specialty care, or ability to obtain certification such as AAHIVP through the American Academy of HIV Medicine, as providing by the agency D:\116100997.doc
