Morbid obesity (or clinically severe obesity) is defined as being over 200% of ideal weight, more than 100 pounds overweight, or a body mass index (BMI) of 40 or greater, at which serious medical conditions occur as a direct result of the obesity. What causes this challenge that adversely affects the health of so many men and women? There are several contributing factors to excess weight or obesity. Obesity can cause increased risk for many diseases and health conditions. These include: Type 2 Diabetes – A long-term metabolic disorder in which the body produces, but resists, insulin. Because of excess weight, obese individuals develop a resistance to insulin, which controls blood sugar. While Type 2 diabetes may occur regardless of someone’s age, gender, or body mass, the disease tends to be more severe in the obese. High Blood Pressure/Heart Disease – Excess body weight often results in high blood pressure (hypertension), which can cause strokes and heart and kidney damage. Evidence shows that the age-related lifetime risk of hypertension in men and women ages 45 to 54 will double as their average body mass index (BMI) increases from 25 to 35. While hypertension may occur regardless of age, gender, or body mass, it tends to be more severe in the obese. Osteoarthritis of Weight-Bearing Joints – Osteoarthritis, one of the most common forms of arthritis, is a chronic condition resulting in a breakdown of joint cartilage. Excess body weight on joints, particularly knees and hips, results in rapid wear and tear, and pain caused by inflammation. Similarly, bones and muscles of the back are constantly strained, causing disk problems, pain, and decreased movement ability. Sleep Apnea and Respiratory Problems – Obstructive sleep apnea occurs when breathing suddenly stops because soft tissue in the rear of the throat collapses and closes during sleep. The greater your excess body weight, the greater the amount of fat pressing down on your chest and lungs. Those who are morbidly obese are likely to have a greater buildup of fat deposits in the tongue and neck adding to sleep and respiratory problems. Reproductive Health – Women challenged with morbid obesity can struggle with reproductive health concerns. Issues such as infertility and menstrual irregularities may occur due to morbid obesity. Fertility issues include possible miscarriage, reduced success with fertility treatments, and polycystic ovarian syndrome (PCOS). Bariatric surgery continues to benefit people struggling with morbid obesity and can save lives. Recent studies indicate that bariatric surgery patients are living longer lives than morbidly obese individuals. Although there are numerous benefits, this surgery is not without risks. It is important to consider these risks and weigh them against potential benefits with an SSM Weight-Loss Institute physician. Bariatric surgery can reduce mortality rates and extend life expectancy. Compared to a person of normal weight, a 25-year-old obese man has a reduced life span, and can expect a loss of about 12 years of life. Bariatric surgery has been shown to improve life span by reducing excess body weight. In a recent study, bariatric surgery reduced overall morbidity and the development of new health-related conditions in morbidly obese patients. Bariatric surgery reduced the relative risk of death by 89 percent with an absolute mortality reduction of 5.49 percent. Bariatric surgery has been proven to impact obesity-related health conditions. Through gastric bypass surgery, patients have shown more than 83% resolution in type 2 diabetes. An ideal program would have at least four components: medical, nutritional, psychological, and surgical. This multidisciplinary team evaluates the patient before surgery and provides education and treatment after surgery. The team also delivers medical, nutritional, and psychological care, and monitors lifestyle changes following surgery. Once you and your surgeon have agreed upon a procedure, your next step will be to set a date. Before Surgery Preparing mentally and physically for surgery. How can you prepare mentally? Bariatric surgery is like other major surgeries. You can best prepare by knowing the benefits and risks of surgery and by closely following your doctor's instructions. To mentally prepare yourself: Understand the surgical process and what to expect afterward. Keep in mind that you’ll never be able to eat the way you did before, and that you’ll have to watch the way you eat for the rest of your life. Talk to people who have had bariatric surgery. Write a letter to yourself and your surgeon explaining your reasons for having bariatric surgery and outlining your plans to maintain your weight-loss after surgery. Start a journal. Record how you feel now, the challenges you face, and the things you hope to be able to do after bariatric surgery. Get a letter of support from your family. It helps to know you have people behind you, waiting to help. How can you prepare physically? Follow the guidelines that your doctor will give you. The guidelines will be based upon your procedure, your personal profile, and other factors. You want to ensure your best outcome, and the guidelines will help you accomplish that. Day of Surgery A healthy new lifestyle begins today. For many weight-loss surgery patients, the long-anticipated day of surgery is an exciting end to a long wait. It’s often referred to by post surgical patients as their “birthday,” because of the life-changing transformation of health they’ve achieved. Preparing for the day of surgery is like preparing for many other surgeries: Your surgeon will have specific instructions for you to follow. A few of the more frequently given instructions include: Do not drink or eat anything prior to surgery, beginning the evening before the day of surgery. Most medicines can be taken the day of surgery, just with small sips of water (however, your doctor will have instructions regarding specific medications). Bring all of your medications with you. Bring your CPAP machine, if you have one, for use in the hospital. Arrive early; some hospitals and surgery centers require patients to be up to two hours early. When you arrive, you will undergo a few preoperative steps. You will change out of your clothes. You will see your nurses, and often, you will see your surgeon as well as your anesthesiologist. An IV line will be started to keep you hydrated and to administer medications such as antibiotics. The preoperative experience is often a short one, after which you will be taken to the operating room. After surgery, in the recovery room, you can expect to have some discomfort and feel the effects of the surgery (this can last for several days). Unfortunately, you may feel worse before you feel better. In the early days after surgery, you may not feel much different. Even though you’ve taken a big step, you may find yourself asking what has changed. There are short-term risks. All abdominal operations carry the risks of bleeding, infection in the incision, blood clots, lung problems (pneumonia, pulmonary embolisms), strokes or heart attacks, anesthetic complications, and blockage or obstruction of the intestines. These risks are greater in patients suffering from morbid obesity. You should be aware that some surgical side effects, such as a blood clot, can be life-threatening. According to the 2004 consensus statement from the ASMBS, the risk of death during bariatric surgery is less than one percent. Life After the Surgery Making a full, positive recovery from your procedure. Bariatric surgery is not a quick fix. It’s an ongoing journey toward weight-loss through lifestyle changes. After surgery, the difference in your body makes it physically easier to adjust your eating and lifestyle habits. Fortunately, you will not have to go through the process alone. A team of professionals will be there to support your efforts. Positive changes in your body, your weight, and your health will occur, but you will need to be patient through the recovery process. Recovery takes effort. The diet is strict. Your body is trying to heal. Your weight is beginning to adjust. The recovery stage of the process becomes easier once you start to see positive changes. The timing varies by patient. Some patients begin to lose pounds within days of their surgery. For others, a couple of months go by before they see noticeable weight loss. With courage and a circle of support, you can make it through recovery. The changes made to your gastrointestinal tract will require permanent changes in your eating habits that must be followed for successful weight-loss. Post surgery dietary guidelines will vary by bariatric surgeon. You may hear about post surgery guidelines different from the ones you receive. It is important to remember that these guidelines will be different depending on the surgeon and type of procedure. What is most important is that you follow your surgeon's guidelines. The following are some of the generally accepted dietary guidelines: When you start eating solid food, it is important to chew your food thoroughly and eat very slowly. It is important to wait two to three minutes after swallowing before putting the next bite of food in your mouth. You will not be able to digest steaks or other chunks of meat if they are not ground or chewed thoroughly. Don't drink fluids while eating. They will make you feel full before you have eaten enough food. Fluids consumed with meals can cause vomiting and dumping syndrome, and can lead to feeling hungry sooner after a meal. Don’t eat desserts and other items with sugar if they have more than 3 to 5 grams per serving size. Avoid carbonated drinks, high-calorie nutritional supplements, milk shakes, foods high in fat, and foods that have no nutritional value. Avoid alcohol. Limit snacking between meals. Maintaining Weight-loss An ongoing journey toward weight-loss through lifestyle changes. Bariatric surgery is meant to help you keep off excess body weight for life. Maintaining weight-loss means a dedicated lifestyle – developing new attitudes toward food, exercise, and even in how you relate to other people. Bariatric surgery is not an immediate fix. It’s a positive, ongoing process of change. Following surgery, your body will give you signals to stop eating before you take in extra calories that would return your excess body weight. It’s what you do with these signals that count. When you develop lasting positive habits, you can achieve lasting weight-loss without dangerous weight-loss/weight-gain cycles. At least two-thirds of patients who have gastric bypass surgery are able to keep off at least 50 percent of their excess weight for 10 years or longer. We offer SSM Weight-Loss Institute patients three procedures: Roux-en-Y gastric bypass [Link] The Adjustable Gastric Banding System [Link] Sleeve Gastrectomy [Link] Please note: Intestinal bypass surgery and stomach stapling are no longer used because of complications as well as failure resulting in weight gain after a few years. What is the recovery time? SSM Weight-Loss Institute patients usually stay in the hospital one to two days in private rooms. Depending on the patient’s work, he or she can return in one or two weeks after weight-loss surgery, as opposed to the average six weeks for traditional surgery. Outpatient surgery for adjustable gastric banding may be available. Is bariatric surgery covered by insurance? We advise surgical candidates to ask their insurance provider to verify coverage. Some insurance companies are now requiring patients to first complete a specified amount of time in a medically-supervised diet program, such as our HMR® Program before approval. Link to Insurance Page How do patients’ eating habits change after surgery? Patients will meet with specialists on their SSM Weight-Loss Institute bariatric team to determine the diet regimen they should follow. Generally, patients are on a liquid diet for four weeks following surgery, but gradually return to eating normally, but in smaller amounts. Can you talk about diet specifically? During the first four weeks, patients eat only protein supplements and pureed foods. In the fifth week, they move on to semi-soft food. And at the sixth week, patients can eat most foods, only eliminating refined sugars and fats. We will recommend a daily calorie total for each patient. This usually falls between 1000 and 1200 calories. How much food will I be able to eat following surgery? How often will I be able to eat? Most patients are instructed to eat 1/4 cup, or 2 ounces, of food. As time goes on, you can eat more (as instructed by your medical team). Most people can eat approximately 1 cup of food after a year or more post surgery. What could happen if I don’t follow one or more of the dietary guidelines? The guidelines are designed to improve the chance of long-term success in weight loss. If you don’t follow the guidelines, you may not lose or maintain the loss of the estimated 70 to 90 percent of excess weight. You may experience complications such as vomiting, diarrhea, or malnutrition after surgery. How soon will I be able to walk? Soon after surgery, doctors will require you to get up and move around. Patients are asked to walk or stand at the bedside the night of surgery and take several walks the next day and soon after. Upon leaving the hospital, you may be able to care for all your personal needs, but you will need help with shopping and lifting, and with transportation. How soon can I drive? You should not drive until you have stopped taking medications (associated with surgery) and can move quickly and alertly. Usually, this takes seven to 14 days after surgery. When can I go back to my normal activity level? Your ability to resume pre-surgery levels of activity depends on your physical condition, the nature of the activity, and the type of bariatric surgery you had. Many patients return to normal levels of activity within six weeks of surgery. Should I use birth control? It is strongly advised that women of childbearing age use the most effective forms of birth control during the first 16 to 24 months after bariatric surgery. Here are some questions patients have asked about maintaining excess weight-loss and lifestyle changes after surgery: Why is exercise so important? When you have bariatric surgery, you lose weight as the amount of food energy (calories) you can eat becomes much less than your body needs. Your body has to make up the difference by burning unused fat or muscle tissue. Your body will tend to burn any unused muscle before it begins to burn the fat it has saved up. Without daily exercise, your body will burn unused muscle, and you will lose muscle mass and strength. Daily aerobic exercise for 20 minutes will tell your body to use your muscles and force it to burn the fat. How much exercise is needed after bariatric surgery? Exercise is an important part of success after surgery. Exercise actually begins on the afternoon of bariatric surgery––the patient must be out of bed and walking. The goal is to walk further every day after that, including the first few weeks at home. You may be encouraged to begin exercising, limited only by discomfort, about two weeks after surgery. The type of exercise depends on your overall condition. Some patients who have severe knee problems can't walk well, but may be able to swim or bicycle. Many patients begin with low-stress forms of exercise and move on to more demanding activity when they are able. Why is it important to drink so much water? When you are losing weight, there are many waste products to eliminate, mostly in the urine. Some of these substances tend to form crystals, which can cause kidney stones. A high water intake protects you and helps your body rid itself of waste efficiently, promoting better weight loss. Water also fills your stomach and helps create a feeling of fullness. If you feel a desire to eat between meals, it may be because you did not drink enough water in the hour before. Insurance Coverage Understanding your weight-loss treatment payment options. It is important to note that insurance coverage associated with weight-loss surgery and treatments varies greatly between insurance companies. Before pursuing any type of professional weight-loss assistance, be sure to contact your insurance provider – not all carriers will cover weight-loss treatments and procedures. If your health insurance does not cover the costs of weight-loss treatments, you will be completely responsible for all of your weight-loss medical bills. Your insurance provider may require that you have prerequisite weight-loss treatments before allowing you to receive insurance coverage for surgery. For instance, some insurance companies require documentation of participation in a six-month physician-supervised weight-loss program, like HMR® (Health Management Resources), in the previous year before weight-loss surgery. Be sure to ask for all the details. Until we receive insurance authorization, or payment arrangements have been finalized, we cannot schedule a procedure. Your insurance provider will notify both of us at the same time regarding your approval status. If you have questions regarding the progress of your request, please contact your insurance provider. Note that the evaluation and insurance/payment verification process may take up to six months to complete. Surgical Treatment Billing Process After you have received treatment at SSM Weight-Loss Institute, we will bill your insurance carrier using the information you have provided. After processing your claim, your insurance carrier will notify us. Note that you may receive an “Explanation of Benefits” (EOB) from your insurance carrier, which will explain how your bill was processed and will also indicate if you are responsible for any outstanding fees not covered by the terms of your insurance. Once the claim is processed by your insurance carrier, we will balance bill the guarantor. Also, we may seek assistance from the subscriber of the policy if we fail to receive a response from your insurance carrier. Note: Not all charges may be included in your bill. Physician billing groups may invoice you separately. Managed Care Plans You should be aware that your plan may require that your Primary Care Physician (PCP), or plan, may be required to authorize your services if you are a member of a Managed Care Plan. We cannot proceed with the process if requested information is missing. 3rd step: SSMWLI staff reviews your packet and checks it for any missing information. If any information is missing, then you will be contacted and asked to provide the missing information. Once your packet is complete, the surgical nurse will notify you that we have the required paperwork and we will request a pre-authorization letter from your insurance company. 4th step: Once SSMWLI receives pre-authorization from your insurance company, a nurse will call and schedule you for a pre-operative class. Your surgery date will be set at that pre-operative class. If insurance denies authorization of this surgery, then the nurse will contact you and share the reason for denial. You do have the right to appeal this denial with your insurance provider; however, the appeal is your responsibility. Clinical Coordinator – A bariatric-trained registered nurse will provide guidance, answer questions, and serve as a continuous resource throughout your entire experience. For Ann, the pounds packed on slowly over a number of years until one day she found her doctor recommending she start taking blood pressure medicine. “I was shocked. My blood pressure had always been lower than average,” says Armstrong. She also suffers from degenerative osteoarthritis, and knew if she didn’t lose some weight she wouldn’t be able to put off knee replacement surgery much longer. “I remember thinking, I’m a young woman. I don’t want to live my life this way.” Armstrong’s doctor recommended she enroll in the HMR® Weight Management program at the SSM Weight-Loss Institute at DePaul. “I chose the most stringent, medically-supervised program because I wanted to lose significant weight to improve my health,” says Armstrong. She was put on a nutritious meal replacement program that also includes daily exercise, weekly visits with a doctor and a weekly support group. “The food is very low-calorie, but I can honestly say that I was never hungry, even from the beginning. And everything tastes good.” The program takes discipline, but Armstrong says the support group is a big help. “We encourage each other and share recipe ideas and new exercise routines. It’s important not to get bored.” Armstrong spends 30 minutes on a stationary bicycle every morning, but she likes to mix it up in the evenings. “I like to do a lot of different workouts like aerobics or Pilates or even Tae Bo. My workouts have gradually gotten longer and more intense.” A little over a year after Armstrong began the HMR Program, she lost 140 pounds. “I never thought I would feel this good again. It’s beyond my comprehension,” she says with a sigh. Armstrong is off of blood pressure medication and doing things she never would have before. “I climb the stairs at work every day now. I can do military-style push-ups. I’ve never in my life been able to do those. And I’ll actually put on a bathing suit. I get in my sister’s hot tub every weekend and I don’t turn down fun social things like float trips anymore.” When asked if she feels like a new person, Armstrong answers with a big smile, “I get asked that a lot. I guess I feel like myself, but even more so. I’m more energetic, more self-confident. I would have never done this if it weren’t for the program, but I’m so glad I did.” Armstrong says her knees are still painful, but she believes losing the weight will buy her more time before she needs knee replacement surgery. She also feels strongly that she was headed for many more health problems if she hadn’t acted when she did. “For me it’s all about being healthy. I don’t ever want someone else to have to take care of me because I didn’t take care of myself.” Blog (To come.)
