Access to BSL3 Laboratory
Pre-requisite
Provide documentation of 6 months (min) of independent work in BSL2 laboratory using risk group 2
agents.
DOJ Process Keiller/GNL only (Completed in parallel to training program)
Contact Campus Police to obtain Select Agent fingerprints
Meet with Dee, JeT, or Sophie to complete SRA paperwork
Bring fingerprints to meeting
Have Social Security number
If not US born citizen, bring:
Passport
Visa paperwork or Green Card
I-95, if on Visa
If naturalized, bring:
Documentation of naturalization
EHS will contact trainee once SRA approved status is granted. Process may take up to 3-4 months. Contact EHS if
6 months has passed.
Occupational Health Process (Completed in parallel to training program)
Meet with Dee, JeT, or Sophie to request Occ Health evaluation
Make appointment with Employee Health
Complete special immunization form, if needed
Review of respiratory assessment forms
Draw base line serum (if signed consent form)
(Bring serum back to EHS as soon as possible, do not shake or invert tube)
Wait 2 weeks before entering BSL3 following immunization
Training Process
Download registration forms found on EHS/Biological & Chemical Safety website
(www.utmb.edu/bof/bc/access_training.asp)
Return completed registration forms to EHS -- before the 1st of each month –
Trainee information
Form 1, Section A and B completed and signed by BSL3 PI and approved mentors
Form 3 signed
Form 4 signed
Incomplete registration packet will result in delay starting training.
Read safety manual and BMBL (most current edition)
Training program will contact trainee to:
Confirm registration packet is complete
Inform of dates and location of next available theoretical class
Set-up pre-assessment tests

Access to BSL3 Laboratory
Theoretical (grades above 96% for a given section will be exempt from final testing on that
section, except with BSC section)
Practical
Report is sent to trainee and PI
Attend theoretical class (2 classes x 4hrs each)
Class may not be attended if pre-assessment has not been completed
Schedule hands-on training (3x2hrs sessions, minimal)
Complete final assessment
Theoretical (passing grade is 80% on each section)
Practical
Final reports will be provided to trainee, PI, and Laboratory Director
Meet with Dee, JeT, or Sophie for final check-off
Meet with Director for facility orientation
Autoclave training
Documentation for autoclaving
A-Test packs
Entry and Exit procedure
Visitor log records
Commence mentorship
Experienced BSL3 user – minimal 10hrs (must be pre-approved by EHS)
Experienced BSL2 user = minimal 20hrs
Mentorship must have begun within 3 months of completing training program with EHS.
Failure to do so will result in retraining.
Provide documentation of mentorship to BSL3 Director for final signature
Independent access will be granted once DOJ clearance is received.
