An agreement by and between LONG S FARMS and __________________________________
(hereinafter referred to as “OWNER”) pertaining to the dog described above.
The terms of the agreement are that:
1.
LONG S FARMS will take into possession the described dog for the purpose of training,
boarding, or day care for the specified period of time as shown above.
2.
OWNER will, prior to enrollment, provide veterinarian records that indicate dog is up to date on
all required vaccinations, including the Bordetella vaccine.
3.
LONG S FARMS will provide adequate shelter and water to assure the physical welfare of the
dog. OWNER will provide the food.
4.
LONG S FARMS will, in the event of sickness of, or calamity to the dog, secure the services of a
veterinarian as specified by the owner. OWNER will be notified before dog is taken to
veterinarian. All vet expenses incurred will be borne by the OWNER. However, if OWNER’S
veterinarian is unable to be reached in an emergency situation, LONG S FARMS will take the
dog to Bellville Vet Clinic.
5.
LONG S FARMS will make a diligent effort to train the dog in the functions normally expected
of a dog having an equal amount of training. However, no guarantee, expressed or implied, that
the effort to train this dog will be successful is a part of this agreement.
6.
OWNER will pay _____________________________for the period of time set forth above.
7.
Payment is due at the conclusion of the training, boarding, or day care. Payment will be in cash
or by check.
8.
The parties hereto agree that LONG S FARMS or their employees or agents will not be held
responsible in the loss of, or the demise, or the injury of this dog for any cause.
