Nurse Practitioner Minor Illness sessions are very different to routine nursing appointments. The Nurse Practitioner treats: Chest infections, Colds,Eye infections, Ear infections(ear wax problems please make appointment with Practice Nurse),Sore throats, Urinary infections,Rashes, Breast lumps, Gynaecological infections, discharge or prolapse, Emergency contraception, Contraception advice/commencing contraception, Implant insertions & removals, Hayfever Unfortunately it is NOT possible to book appointments with the Nurse on-line. Thank you. See our Emis Access User Guide for further information From 1 April 2008 staff can no longer accept verbal requests for prescriptions. They must be made in writing, either by post, request form, e-mail or on-line. We now offer late appointments (up to 8.00 pm) on Tuesdays. You can book these via EMIS Access or at reception. TEXT MESSAGE SERVICE: . We now offer a text reminder service for appointments and health checks in the near future. If you would like to use this service please complete a consent form at the practice. Requests for repeat prescriptions can be posted to the surgery, handed into reception,or ordered online. All requests must be made in writing or online. Prescriptions will be ready to collect after 4pm the next working day or posted if a SSAE (Stamped Self Addressed Envelope) is provided. Any prescriptions requested after 6pm on Friday will not be seen by staff until Monday morning, therefore will not be ready for collection until after 4pm Tuesday. To order your repeat prescription click on the online prescriptions tab above. Online Prescriptions Please come to the Surgery in person and speak to the Receptionists. You will be given a Registration Letter with your unique registration details that you should keep safe. Enter the information from the Registration Letter into EMIS Access Registration page by clicking on link below. Click Here to order your repeat prescription online EMIS Access is a web based system that allows: Appointment booking/cancellation Repeat Prescription requests Messaging - send messages directly to the surgery. To use the service you have to be registered with the Surgery and you will need the following: Our Practice ID number your Personal Identification Number (PIN) your Password your NHS number Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. Out of Hours If you need emergency treatment when the surgery is closed please ring 01543-674477 Your call will automatically be diverted to the out-of-hours provider. If for some reason your call is not automatically diverted you can ring BADGER, our out-ofhours provider, direct on 0300 130 3030 Minor casualties can be seen at the Minor Injuries Unit at Samuel Johnson Hospital in Lichfield (tel: 01543-412900). A member of the Carers Association South Staffordshire (CASS) will be able to provide useful advice and information. WHAT IS CASS? CASS support unpaid carers of any age in southern Staffordshire, who are looking after a friend or relative, who because of disability, illness or the effects of old age cannot manage at home without help. CASS is a registered charity which provides confidential advice, information emotional support and a range of other services to Carers living locally. Dr Ward will be reducing from full-time to part-time at this time, although the overall number of GP appointments each week will be higher than it has been previously. Identifiable information about you will be shared with others in the following circumstances: To provide further medical treatment for you e.g. from district nurses and hospital services. To help you get other services e.g. from the social work department. This requires your consent. When we have a duty to others e.g. in child protection cases. Anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care. If you do not wish anonymous information about you to be used in such a way, please let us know. Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff. Occasionally staff from the PCT may be allowed access to records to verify the procedures that we use and ensure that we record information according to national regulations. Whilst a random selection of patient records have to be viewed to confirm that our procedures are appropriate, PCT staff do not make a record of any personal information that they view. If you do not want us to allow PCT staff to view your records please state this in writing and your records will be updated accordingly. Tracy qualified as a Nurse Practitioner in 2007 and holds a minor illness session every day. Tracy is able to treat and prescribe for a wide variety of illnesses including coughs, colds, ear infections, eye infections, sore throats, urinary infections, rashes, breast lumps, gynaecological infections and emergency contraception These clinics are not for general nursing appointments (ie: smears, pill checks or blood tests etc). Nurses Mrs Tessa Tunley Tessa offers appointments for all routine nursing care, which includes smears, travel vaccinations and smoking cessation Healthcare Assistants Deborah Deborah runs 3 sessions each week. Deborah provides various services including blood pressure checks, new patient checks, well person checks, phletobomy (blood tests), ECGs and spirometry. Practice Management Mrs Lisa Eales Practice Manager. Mrs Lisa Eales is responsible for the running of the practice. She has been involved in practice management since 1992. Mrs Karen Assistant Practice Manager Felton Administration Jo Jo is the practice Secretary and is repsonsible for NHS referrals, private referrals and using the Choose & Book system. Sue Sue is responsible for administration including patient registration, insurance forms and the cervical cytology recall system. Sarah Sarah is responsible for ensuring that the practice achieves certain targets set for practices by the Department of Health. Counsellors Mrs Rhoda Rhoda is an accredited member of the British Association of Wharton Counsellors and Psychotherapists. Rhoda has worked at the practice since 2003 and provides counselling for patients who are referred to her by a doctor.
