I ______________________________, a student of the School Driver Ed. Program agree to:
1) Provide a copy of my Birth Certificate to the Career/Transition Advisor and fill in the forms.
2) Find out when the Driver Education in school classes are to be held and where. (Get a
Schedule from the Teacher and make sure you can attend).
3) Attend ALL of the in school classes and in car driving lessons. I understand that
attending all of the classes is necessary to complete the Driver Ed. Program and to achieve a
Graduated Drivers License. Understand that sometimes some of the classes might be after
school hours.
I understand the following:
 If a student misses “more than two classes” they will be discontinued from this
Driver Ed. Program.
 If the student misses a class it is up to the student to ask how they can make the
class up. This will be only be possible if the Driver Ed. Teacher will provide a
make up class at a time that is convenient for the Driver Ed. Teacher.
 If the student does not complete or pass their first Driver Ed. Program that
they are enrolled in, they may try one more time. After the second try the
student will be advised of other options.
 I understand that Summer School is not an option for Driver Ed. training
 An oral exam may be available if the Driver Ed. teacher recommends it.
