If you are under the age of 18, you must have a parent or legal guardian complete this
Release Form.
As a parent or guardian (The Contributor) of a Minor who wishes to contribute a literacy, you have been
informed that the purpose of the Digital Archive of Literacy Narratives (DALN) is to collect people’s literacy
stories and related literacy artifacts (e.g., photographs, poems, texts, home videos) and to store these in a
permanent collection which members of the public can access, study, and learn from.
As a parent or guardian of the Minor, you have been informed that the materials the Minor wishes submit
to the Digital Archive of Literacy Narratives will be posted on the Project’s Web site (unless they contain
material considered inappropriate by the DALN Board).
As a parent or guardian of the Minor, you have been informed that you can choose whether or not
to allow the Minor’s name to be made public on the DALN. You understand that you can also
decide not to answer any questions that are presented on the DALN web site. However, once you
have submitted the Minor’s materials to the DALN (via the web or postal mail) and agreed that it
can be publicly posted, the information you have provided cannot be removed from the DALN.
As a parent or guardian of the Minor, you have been informed that although the DALN Board
members will have access to some information about contributors (such as their age and contact
information), this information will only be used for contact purposes. This information will not be
used for research purposes or made public unless you give permission for this information to be
publicly posted.
As a parent or guardian of the Minor, you know that you can assign the copyright to the Minor’s
narrative and materials to the DALN using our Deed of Gift form that allows the DALN to specify
how the materials can be duplicated, distributed, and used.
As a parent or guardian of the Minor, you have been informed that all the materials on this web
site will be made available to the public. So, the Minor’s narrative could be used by people in
ways that go beyond your original intent.
As a parent or guardian of the Minor, you affirm that this literacy narrative and the additional
materials attached to it are the Minor’s own original work and that the Minor owns all necessary
rights to this work. As a parent or guardian of the Minor, you also affirm that this work does not
defame or otherwise violate the rights, copyrights, trademarks, privacy, or reputation of any other
person.
As a parent or guardian of the Minor, you agree to hold the Digital Archive of Literacy Narratives
(DALN) harmless against any claims asserted by third parties who say that the Minor’s work
violates their rights or from any breach of promises the Minor may have made in this document.
As a parent or guardian of the Minor, you understand that you are responsible for any costs
incurred by the Digital Archives of Literacy Narratives (DALN) for defense or settlement of thirdparty claims, including but not limited to the Digital Archives of Literacy Narratives (DALN)
reasonable attorneys' fees.
As a parent or guardian of the Minor, you affirm that you have read, acknowledged, and agree to
be bound by each of the terms and conditions of this agreement.
If you do not agree to be bound by each of the terms and conditions of this agreement, you
understand that you should not sign and submit this form. In that case, the Minor will not be allowed
to contribute to the DALN.
