This is subject to change in the event of annual leave or study leave. Appointments can be pre-booked up to 4 weeks in advance subject to availability. Half of all the day’s appointments are kept for booking on the day only – this is in response to our patients’ requests. Please phone as near to 8.00am as possible if you need to be seen that day and please note that you may not always see your own GP or be able to select a time. If you need to be seen, please tell the receptionist and she will add you into a surgery – you may not been seen by your own GP or be able to specify a time. Pre-Bookable Appointments There are pre-bookable appointments available to book one month in advance, but these do get taken very quickly. We have also introduced a number of appointments that are released 24 / 48 hours in advance - these may not be with your normal doctor. PLEASE NOTE APPOINTMENTS FOR THE DAY WILL NOT BE RELEASED UNTIL 8AM, WHEN THE TELEPHONES ARE SWITCHED OVER, THIS INCLUDES ANY ONLINE APPOINTMENTS THAT ARE HELD BACK FOR ON THE DAY BOOKINGS Home Visits Please come to the surgery if you are able to as all facilities are to hand. If however you are too ill to travel you may ask for a visit by phoning reception. The doctor will discuss matters with you by telephoning you by return. It helps us a great deal if a request for a home visit is received by us before 10.30 am although we realise this is not always possible. Please note that the receptionist will ask for details as to why you need the visit - this is to help the doctor to prioritise their workload. Please be aware that it will be the Duty Doctor who makes the visits, so you won't necessarily see your normal doctor Test Results The practice has a strict policy regarding confidentiality and data protection. We will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results. Please also note that test results are given out after 3pm in the afternoon, as the telephone lines are very busy in the morning. Each GP that is working will have limited telephone appointments available. You will be asked to leave your name and contact landline number (where possible), a brief idea of what the call is about, if you are happy to tell the receptionist and you will be advised of a two hour time slot that you will be called back in. Please be aware that sometimes emergencies do crop up and the Doctor may be unable to call back during that slot - this is normally unavoidable and we apologise in advance. Repeat Prescriptions Repeat prescriptions cannot be taken over the telephone, as this can lead to mistakes with specific wording and dosage. It is important for us to eliminate the possibility of error, this safeguards you as the patients. This change is in line with other local practices. Repeat prescriptions will be processed in 2 working days. We will take repeat prescriptions over the telephone for patients who are immobile/housebound or terminally ill, A box is available in the reception area for repeat prescriptions to be placed in. URGENT Requests We are now unable to provide urgent on the day requests - it is the patients responsibility to ensure they order their medication in good time. If you cant get into the surgery, then you can order online; if you have lost your slip then please drop your details along with the names of the medications you need into the surgery or order online. If you have no medication left, if you take your empty boxes to your regular pharmacist, they will issue a three day supply or a number of medications can be purchased from the pharmacist. Warfarin Tablets If you are ordering your warfarin medication, then we need your anti-coagulant book in, so as we can take a copy of your last result. This is to ensure that the correct medication is being given, as the hospital still retain the responsibility of dosing for patients, but have passed the responsibility for prescribing to the GPs now. A prescription will not be issued without the yellow book, or a photocopy of your last result. Online Prescriptions This is for routine prescription requests only, as the mailbox will only be checked once a day. Please do not send any medical questions via this form. Click here to access the online Repeat Prescription Form Prescriptions Charges and Exemptions Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs). The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication. NHS charges From 1 April 2009, the charges are: Prescription (per item): £7.20 12-month prepayment certificate (PPC): £104.00 3-month PPC: £28.25 If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is £7.20, whereas a three month PPC will cost you £28.25 and a 12 month PPC £104.00. Any problems please telephone the surgery. Clinics & Services Diabetic Clinic All our diabetic patients are seen regularly bi-annually by the GP or Clare . Insulin dependant diabetics have their care shared between the hospital and the practice GPs but those who are managed with tablets or diet alone are managed entirely by the practice. The incidence of diabetes is increasing in our communities and it is essential that this condition is managed well in order to avoid the long-term complications of heart disease, stroke, peripheral vascular disease, eye and kidney complications. At the clinic patients are first seen by Clare, our practice nurse, who does appropriate blood tests and checks weight, blood pressure and urine. She also checks other risk factors such as alcohol and smoking status, then it"s on to the doctor who looks at the overall control of the diabetes and weight and blood pressure, and who instigates changes in medication and encourages a healthy lifestyle. The legs are also checked carefully for diabetic problems involving the nerves and blood vessels. It is vital that in this condition patients take responsibility for their health, and take on-board the health advice that we try and tailor to each patient. With this successful partnership we hope to give diabetic patients a better quality of life with a long lifespan. Please Note: You need to make an appointment for your routine blood tests 2 weeks prior to your diabetic review. Asthma Clinic This is a clinic run by Diane who has an expertise in asthma. Patients with chronic bronchitis are also seen in this clinic and there are many patients who have a combination of both of these conditions. The aim of the clinic is to allow the patient with the aid of education and treatment to maximise their health and quality of life. Chronic bronchitis is essentially in its purest state a disease, which is caused by smoking. The aim of treatment is to persuade and support the patient to stop smoking, as this is the only intervention, which will halt the further deterioration of this condition. Some patients also have a degree of asthma and are able to have this element of their condition treated and inhalers certainly make life more comfortable easing to an extent the main symptoms of cough, wheeze and breathlessness. We have also the ability to do within the practice, a test called Spirometry. This is an instrument, which can tell us what the severity of the chronic bronchitis is, and help us to distinguish chronic bronchitis from asthma. Asthma is an eminently treatable condition but asthma sufferers tend to underestimate the possible impact of their disease and require regular review appointments to bring them abreast of new treatments and to encourage them to take regular preventative inhalers to control their asthma in the best possible way. We hope that with most patients they will learn how to modify their own treatment to respond to deterioration or stability so that they remain on the least strength of treatment. Depending on the severity of your asthma, you will be seen annually or bi-annually for your reviews. Cardiovascular Clinic This clinic is run by a practice nurse and by the GP. It encompasses the regular review of all patients who have a range of conditions including hypertension, heart disease, strokes and poor circulation. The National Service Framework has set many targets for the monitoring and treatment of these conditions which include blood pressure targets, and targets for smoking cessation and lowering of other risk factors which are very important to these conditions such as levels of cholesterol. Many patients have multiple risk factors which have to be addressed if we are to be successful in reducing the incidence of heart attack and stroke in our practice. Diet and exercise are other important factors and we are constantly promoting these. If you prefer to come in at a quietier time, then lunchtimes between 1 - 2pm (apart from Wednesday's) or between 6 - 7.20pm on a Wednesday evening are good times (when Dr. Campling is doing late surgery). The machine is very simple to use and you do not need to roll your sleeve up and there is no need to remove jumpers or cardigans. Take a seat in front of the machine. You can choose which arm you use Insert into the slot - you will feel a dip, where your elbow should be Press the start button You will feel the cuff tightening, as you would if one of the clinicans were performing this test The machine will register your recording and print off a record of this The cuff will deflate Take the slip to the receptionist, who will ask you to put your name and date of birth on to the top of the slip This will then be passed to your GP to look at For patients that attend twice yearly for hypertension / chronic heart disease clinic, you will be asked to use this machine, instead of your six monthly simple blood pressure appointment. Have a go next time you are in and let us know what you think Patient Registration Patient Registration When you register at the practice you will be asked to complete a registration form and also a questionnaire, this is because it can take a considerable amount of time to receive your records. We MUST receive your registration form and fully completed questionnaire, along with your proofs of identity before we can register you. You must bring along one proof of identity and one document that confirms residency from the lists below. Oct 2013 - We are pleased to announce that building works will commence later this month on the current Poplar Grove site (21st Oct). The first phase will be the new car park and this will take approximately nine weeks. We will keep you updated as the building works continue Stop Press..... 21st August 2013 Contracts for the land have now been exchanged and planning permission granted. Work should start on site at the end of September / October 2013 Over the past five years, we have explored options with the local Primary Care Trust (PCT) to see how and if we could extend our current premises, as we have literally out-grown them and it can no longer meet the needs of a growing and developing practice. Currently, all the clinical staff have to hot desk and move from room to room. A merger would bring with it the opportunity for developing services, where patients can be seen in the community, rather then having to attend hospital for some of their investigations. The Partners and nursing staff would be able to share their clinical knowledge and skills which would be for the benefit of patients as it is becoming clear that small practices are becoming unsustainable because of the demands imposed upon them. The merger is not just attractive to the practices in clinical planning and management terms; it also allows the growing patient lists of over 4,300 at Broughton and 13,000 at Poplar Grove to be catered for in a new, efficient purpose built single building that minimises space loss for circulation and service areas. This is a more effective space solution than maintaining two buildings. The practice merger and the enlarged building provide solutions to these issues, including succession planning. The opportunity for the practices to come together in one building provides a number of simultaneous benefits, while offering new and improved facilities that meet all health & safety, clinical & access requirements & guidelines, including Care Quality Commission registration & infection control standards. As well as providing the existing practice populations with quality primary care facilities that are currently needed, the project represents a cornerstone in the strategic development of services in Aylesbury. This is because the rising population numbers require all practices to provide more facilities over time, as part of a rolling programme. Both practices consulted with their patient poplulation in September 2011 and various meetings have been held with Local Councillors, the Overview and Scrutiny Committee, AVDC Planning Department and the PCT. An application was submitted to AVDC on the 19th July and acknowledge on the 30th July. We should receive a decision on the application by the 24th September. The plans submitted have been updated and are available to view at the practice. The Government have included children born between 2.9.2009 and 1.9.2011 to the flu vaccination programme this year. You will be receiving a letter from us as these will be administered in a seperate clinic, unless your child currently has the vaccination because of an underlying condition. This is part of a rolling programme and will be extended to older children as the programme gets rolled out over the country Shingles vaccinations will be offered to patients aged 70 ys or 79 yrs as of 1st September 2013. This is a rolling programme and will be offered to patients who have turned 70 years old, for a while it will also be offered to those who have turned 79 years as part of a catch up campaign. This will be given as a single injection and can be given at the same time as your influenza vaccination. The Summary Care Record is meant to help emergency doctors and nurses help you when you contact them when the surgery is closed. Initially, it will contain just your medications and allergies. Later on as the central NHS computer system develops, (known as the ‘Summary Care Record’ – SCR), other staff who work in the NHS will be able to access it along with information from hospitals, out of hours services, and specialists letters that may be added as well. Your information will be extracted from practices such as ours and held on central NHS databases. As with all new systems there are pros and cons to think about. When you speak to an emergency doctor you might overlook something that is important and if they have access to your medical record it might avoid mistakes or problems, although even then, you should be asked to give your consent each time a member of NHS Staff wishes to access your record, unless you are medically unable to do so. On the other hand, you may have strong views about sharing your personal information and wish to keep your information at the level of this practice. Connecting for Health (CfH), the government agency responsible for the Summary Care Record have agreed with doctors’ leaders that new patients registering with this practice should be able to decide whether or not their information is uploaded to the Central NHS Computer System. For existing patients it is different in that it is assumed that you want your record uploaded to the Central NHS Computer System unless you actively opt out. 26th November 2012 All patients age 16 and over will be receiving a pack over the next ten days or so asking them if they wish to have a summary care record. The details that this record will comprise of will be: Name, address, date of birth and NHS Number Your current medications and any acutes you may have had in the past 12 months Any allergies Any adverse reactions If you dont want this to happen YOU MUST COMPLETE THE OPT OUT FORM SENT TO YOU AND RETURN TO RECEPTION WITHIN 12 WEEKS.
