INITIAL EACH NUMBER AS YOU READ AND AGREE
1. Arizona Small Dog Rescue accepts payment in cash or credit card ($10 fee) ONLY.
CASH ONLY AT ADOPTION EVENTS.
2. We DO NOT offer refunds for any reason (see #6)
3. You can return the dog for EXCHANGE of another AZSDR dog for any reason (except
Illness) within 10 days of adoption date. If no available dog matches at the time of
return, you have a credit of your adoption fee for 1 month. If credit is not used within
one month, then the fee will be considered a donation to AZSDR.
4. Your adoption fee is covering the cost of spay/neuter/parvo/distemper vaccine/rabies
vaccine (if old enough) and vet check ONLY. Anything beyond this is your responsibility.
5. Adopter agrees that this dog looks healthy at time of adoption.
6. If your newly adopted dog begins coughing, sneezing, or showing other signs of kennel
cough, you may use your free vet exam to get a prescription for antibiotics. The dog you
are adopting HAS BEEN EXPOSED to kennel cough/upper respiratory infection, as most
of our dogs are saved from high kill shelters. If your current dogs are not vaccinated they
will likely get it as well. Please make sure your dog at home is fully vaccinated with
bordetella/parvo/distemper/rabies before adopting a new pet. All rescue dogs are
exposed to things like parvo, distemper, coccidia, giardia and more. We do not cover
the cost of kennel cough, or any other illness your new dog may get. If your adopted
dog tests positive for parvo with-in 14 days of adoption, please contact us for a full
refund of your adoption fee or for information about treating the parvo virus.
7. AZSDR is a non-profit/no kill shelter. We ALWAYS take our dogs back for any reason
throughout their lives. You CANNOT give/sell this dog to ANYONE for any reason. You
MUST return the dog to AZSDR if you are unable to keep this dog for the dog’s life. You
must contact us before euthanizing any dog adopted from us if you have had the dog for
less than a year. If you do give/sell this dog to someone, you WILL PAY all court fees,
and any other fees that we incur to get dog back to AZSDR.

8. If you do not understand and agree to this contract DO NOT ADOPT A DOG FROM US.
This contract is not negotiable and you must agree to and follow this contract.
9. AZSDR has the right to do a home check before or after your adoption to make sure the
dog is in the best home. AZSDR has the right to remove this dog if not being provided
with the proper care.
10. AZSDR does not adopt out dogs as outdoor pets. All of our dogs MUST be indoor family
members.
11. Adopter agrees to hold harmless AZSDR and anyone working with or volunteering with
AZSDR. Adopter cannot sue AZSDR or anyone involved with AZSDR for any reason.
12. We do NOT guarantee health, age, breed, temperament, or training of this dog. We
can only tell you what we were told by the dog’s previous owner or by the shelter from
where the dog came, or from the veterinarian who examined this dog.
13. Adopter agrees to put a tag on dog today, and if dog is lost to notify rescue
immediately.
14. Work and training will be needed with any dog coming into a new home. This dog will
be scared, confused, and will need time to adjust and learn the new rules and wants of
the owner. Adopter agrees to work with the adopted dog and give every effort
including hiring a trainer before returning the dog for behavioral issues. Adopter
understands that dogs can chew, bark, dig, soil in the house, and exhibit other
undesirable behaviors unless properly supervised, and trained.
15. IF YOUR DOG falls ill after the adoption contract is finalized (at time of signing), YOU
are RESPONSIBLE for its ENTIRE veterinarian care. If for whatever reason, you choose
not to treat your dog, you will return the dog to AZSDR and forfeit your adoption fee
and waive all rights to this dog.
