 WRBHO News and Updates – Adele and John
 Review of WRBHO Program Goals- Adele
 Review of data being collected and reports being prepared to support goals – Anne
Wilder and John Lee
 Overview of practices that support an effective inpatient to outpatient transition- John
Lee and Chris Mangione
 Set charge for the sub-regional meetings to follow - Adele
o Provide feedback concerning your experience with the WRBHO
o Begin the development of a cross-system plan to impact inpatient /outpatient
transitions
Sub-Regional Meetings – 4 Regions – 2:15 to 4:00
Placement by self-identification:
- Chautauqua County Region
- Erie County Region
- Finger Lakes Region
- Monroe County Region

Breakout groups – multi-stakeholder, by region (about 15 individuals in each group) -- will
develop responses to the following questions:
o Provide feedback on your experience with the WRBHO: What has gone well? What
would you like to see improved? What has been the impact to your organization?
o
As you consider the current inpatient /outpatient transition in your community:
 What is working well?
 What should be working better?
 What are the barriers to a more effective inpatient / outpatient transition
process?

What are the 3 things your group would recommend changing in your
region in order to have the most significant impact on this transition?
What should each of the following groups be asked to do in order to achieve
the desired change:
 State and County government
 Peer and family groups
 Inpatient & outpatient service providers
What information would you track in order to know that the system is
either working or not working

Each Breakout Group will report out to the Sub-Regional group as a whole, providing the
following information:
o The top 3 things the group recommends changing in the region in order to have the
most significant impact on the inpatient/ outpatient transition
o The number 1 task for each of the following stakeholder groups in order to achieve
the desired change:
 State and county government
 Peer and family groups
 Inpatient service providers
 Outpatient service providers

Sub-Regional Group will agree upon three steps they can take together between now and
the next meeting (3 months) to positively impact the inpatient / outpatient transition.
